What Causes Severe Pain In Neck,shouler And Arm?

Question: I have sudden severe throbbing (had to go to emergency room 3 times in 2 weeks) in rt. side of neck, and on rt. shoulder and rt upper arm. Throbbing with excruciating pain like nothing one could live with. I even asked " if they can't remove the pain- could they take my arm off " It is removed by I V pain medicine or a pain pill or two then it is gone for an unknown amount of time - maybe a day or two, can't figure out if it is food ( carbs or suger) or activity , or disease that causes it. Pain does not go past elbow. Above elbow the skin is numb , not the muscle. I am 60 on no meds ( don't like or believe in Pharma ) only take vitamins and all tests say I am as healthy as a log. We have only done the most minor heart tests and no cancer tests. All of this started in October with severe pain in left armpit and then became accompanied by left shoulder going a little into chest- was intermittent with strong pain pretty regular. That annoying , intermittent throbbing pain came and went until February. I went for a physical and blood test ,They said everything looks great. I am pretty active and in great health and am never sick. It was Suddenly gone and then a sore neck on rt side appeared and stayed for months - just a boring bothersome ache, then it became unbearable, throbbing, and excruciating and included the neck shoulder and upper arm , ( rt side only) and is still present ,. skin to touch is numb , muscle is not.Have had MANY ex rays and every Dr says about past Dr." why did they take an ex-ray, they don't show anything" Had an MRI of spine on neck ,showed swelling. Help- don't want to live in pain pills forever. When the attacks come there is also a pain in my head on lower rt side. Help me please, Diane ( anesthesiologist put me on steroid packet for a week with gabapentin 180 with refils) have not noticed any improvement- am now a walking pharmacy and can pick any pill I want ! I go to bed with a gabapentin ,if in pain, if I wake up at night with more pain I take a hydracodine if that doesn't work I go and sit up on the couch and that posture seems to give some relief. I don't know how to upload or pdf or I would, because I have everything including cd's of exrays and MRI. They did standard blood tests at the 3 emergency rooms and every test came back great( they said) I am 60 and weigh 110 lbs take only 12-15 vitamins a day, no other meds -am healthy - should they be testing me for diabetes or thyroid disease or cancer ,this investigation has been going on since Feb and no one has recommended anything . ( I have a large protrusion that has grown on my back in the last year and a half, it is the size of an entire hand cupped out right in the middle of back on left side of spine - no one has recommended MRI of lump and is just assuming it is scoliosis - does scoliosis just appear in a year ?

Looking at the clinical history and symptoms, my opinion is as follows:
1. As the pain starts from neck and is radiating to arms, it could be compression of nerve from possible vertebrae.
2. If the pain increases with work and reduces with rest, spondylosis of cervical region is a possibility.
3. Steroids block will help for short period.
4. Neck muscles strengthening, cervical collar, painkillers, reduced work in hands and limiting extra weight work should help.

Rest does not decrease pain - only pain medications and muscle relaxers and time. I have been extremely active my whole life , with lots of bending , I have done lots of wall painting , sanding , scrubbing, furniture moving, bed making, furniture lifting , gardening, that involves heavy lifting and digging and shoveling why now can I only do nothing without causing pain. Do you not find this strange? Why has that lump just appeared on my back? What can cause that to suddenly happen? If my pillow touches my arm it will be throbbing in 5 or 10 minutes . I can not wear a bra or the bra straps will cut off my circulation and the arm will start to throb . Why has the skin on my arm become numb?

Hi,
Thanks for asking again.
My answer is as follows:

1. No, it is not strange. As age related degeneration in vertebrae along with muscle weakness. It is a wear and tear disease. The nerve due to vertebrae compression (possible disc prolapse) will get compressed. These are the nerves which carry sensation. If the compression is more, it will send it as a pain signal, or as numbness.

2. Lump on your back, within one year, is difficult to say without an MRI. However, if it is hard like a bone, it could be scoliosis (not known to occur so fast without accompanying bone disorders like osteoporosis). If it is soft, it could be a lipoma like lesion. More details necessary for opinion. Size appears to be large.

3. Any abnormal position of cervical spine, or any position change can trigger the nerve compression and cause more pain.

4. Lifting the arm above the level of neck is going to cause severe pain. And it is not the blood circulation that is getting cut, it is the nerve getting more compressed.

5. Numbness is again due to nerve compression.

You can in relation to pain do:
1. Use a cervical collar for most of the time, check your size and should support you well. If not sleeping, bathing or eating, wear cervical collar.
2. Avoid any work, other than having your own meals, wearing clothes, using restroom. Even for bathing, use showers. and after that take rest. No lifting anything
3. Avoid pillow and try to use a flat surface while sleeping.
4. Take the steroid block for now, and then you can concentrate on improving your muscle strength
5. Back exercises and neck exercises, slowly to improve muscle strength.
