What Causes Severe Pain During Night On The Inflamed Right Shoulder?
Question: Hello Doctor, The problem I am mentioning below is about by auntys health issue. She is in her 50s now. She had her rheumatoid arthritis problem for past 5 months.. Now for the past one week she is suffering from severe pain during night on the inflamed right shoulder while sleeping... sir, she had T-sazz 1000mg (morning and night),oncotrex weekly once for past 3months now the doctor advised to take hydroquin (morning),T-saaz and T-Ezcat during (night)..whether the newly prescribed tablet can be taken for treating Rheutamoid arthritis....i have to know the details regarding hydroquin,sazz and ezcat..Please guide us which is the best medicine without side effects. Blood test report SERUM ANTI C C P (cyclic citirullinated peptide) IgG Result= 0.90 RU/ml less than 5.0 (ELISA METHOD)

Your aunty is getting combination of Disease-modifying Anti-rheumatic Drugs (DMARDs)and non-steroidal anti-inflammatory drugs (NSAIDS).
Earlier she was only taking Disease-modifying Anti-rheumatic Drugs (DMARDs)drugs.
In latest regime she is receiving additional NSAIDS.It is needed because she was not responding well only with DMARDs.
Combination of DMARDs and NSAIDS are the latest treatment protocol in cases not responding well with DMARDs.
Hydroquine and Sazz are DMARDs whereas ezcat is NSAIDS.
Most of the medicines have some side effects but dose is optimized in different cases.
She is getting right medicines in latest protocol and she should continue it.
If she still complains of pain then steroid may be added,but it is generally used very cautiously due to side effects.

Thank you doctor for your valuable answer regarding seronegative arthritis problem.I have another doubt please give me suggestions.. after she had T.HCQ 100mg and T Ezcat 90mg midnight she have some ear and heart beatings .. whether it is advisable to take this tablet in night time. also for pressure she is taking selokenXL25,cilnipar in morning..heart beatings in night time and finding restless in morning not able to do work is due to whether pressure,tablet or arthritis pain …If she have sazz she don’t have any problem but only after having T.HCQ and T EZCAT eye is getting swollen, heart beatings less only..From this she can take saaz alone or T.HCQ and TEZCAT is compulsory or the tablet dosage have to be reduced or the pressure tablet have to be changed.
Hello Doctor, The problem I am mentioning below is about my friend health issue. She is in her 28s now. She have hair falling problem for the past 3 months. As per doctor advice she use to take hairbless in night time.No improvement still she have that problem.she also have dandruff.whether she can apply any oil or tablet have to be changed.

Her problems are related to side effects of HCQ.She may need dose adjustment of HCQ or switch over to other DMARDs.She should consult rheumatologist.
About your friend's problem,hair fall may be due to may reasons like stress,dandruff,hypothyroidism etc.She need proper evaluation.
If there is any stress factor,it should be relieved.
TSH level should also be measured.
She may use selenium sulphide containing shampoo like selsun for dandruff.
She is rightly getting hairbless.She should continue it.Topical Minoxidil & Aminexil combination can also be applied over the hair roots.
