Posted on Mon, 10 Aug 2015
Question: Hi, I am a 66 year old female with moderate to severe pain on the outside of my neck near the clavicle area for 6 months now. I have been seen my an ENT who scoped inside my throat and found nothing wrong. I have GERD but it is controlled very well by omeprazole twice daily. I have chronic cervical disc disease and arthritis on the right side of the inside of my neck. I have also seen a GASTRO Dr. for treatment of this pain but he did not know what it was. Can you help me to identify this pain and possible type of Doc. who could treat me. Thanks, XXXXXXX XXXX
Hi, Please understand, I am suffering in pain, and just would like to know if there is any pain relief and/or diagnosis available to me. Thanks, XXXXXXX XXXX

Brief Answer:
Pain is directly related to cervical disc disease.

Detailed Answer:
Hi Mam,

I understand your concern.

The pain in your case is directly related to the cervical disc disease.

GERD or ENT issues has nothing to do with it.

I advise you to get an MRI scan of the cervical spine in order to assess the severity of the lesion..

Definitive relief can arise out of surgical procedures like Laminectomy or Discectomy.

Consultation with an expert Radiologist and Neurologist will help.

Meanwhile pain can be controlled with analgesics like Ultracet can be used once a day.

Post your further queries if any.
Thank you.

Thank you so much Dr. Heijebu, I had also thought of the connection with my cervical spinal disease. I do have an appt. at the OSU Spine Center, so now I know I am on the right track. Thanks again, best, XXXXXXX XXXX

Hello Dr. Heijebu, My outer throat pain is worse now than when I first wrote to you. It gets worse as the day progresses. My appt. at the OSU Spine Center is not until Aug. 13th(the earliest they could do), so I did call for an appt. with a different neurosurgeon who did back surgery on my husband 2 years ago and really helped him, but they have not called me back yet. Sometimes I feel like I am going to choke to death, esp. at night. Would it be ok for me to go to an Urgent Care or the ER room if it gets worse, this is a combo of pain, constriction, and pressure outside of my throat. I have talked with my GP, but he thinks it is GERD related, but I don't think it is. Any suggestions would be welcome, Thanks, XXXXXXX XXXX

Brief Answer:
Seek an Urgent medical care.

Detailed Answer:
Hi Mam,

I am sorry for the delayed response.

Its very important that you seek an early medical care pertaining to the sensation of pain and chocking in the throat.

GERD usually will not cause such acute symptoms .

But its very important that you get an Upper GI endoscopy to rule out GERD or find the cause for upper aero-digestive discomfort.

So finally I conclude that you go for an early medical care by consulting a Critical care specialist Or a Gastroenterologist.

Further management is based on the test results.

Meanwhile a course of antibiotics like Augmentin 625 mg one tab q12h for 3 days will help to control any pharyngeal infection.

Analgesics like Tramadol 50 mg one tab at bedtime for 3 days will help to relieve pain.

Consult your doctor with regards to these as well.

Post your further queries if any.
Thank you.
