Brief Answer:
She needs to present to an ER for an assessment

Detailed Answer:
I'm so terribly sorry that your daughter has not sought medical attention for lack of insurance. Has she not signed up for an insurance plan under the "Obamacare" Health Plan Directive. She should be able to obtain this form of insurance coverage mandated by the Federal Government so long as she qualifies under their financial criteria. Here is the website for the IDAHO Healthcare insurance exchange under the Obama Healthcare mandates that she should be able to either sign up in order to purchase minimal health care coverage...or at least call to inquire as to where she can get help if uninsured and unable to obtain any insurance despite government mandates.

Or call 800.508.6754 for more information if the above link doesn't provide enough answers to questions or a way for your daughter to sign up for the Affordable Healthcare Act.

As far as what's going on I highly suggest that your daughter visit the ER of any local or county hospital with or without insurance coverage. If she is indigent then, she will likely be rated for financial aid at least by the hospital. I don't know of a single hospital in the U.S. that would turn someone away with your daughter's symptoms if they are without healthcare coverage but suffering from a systemic disease entity such as what you're describing without some form of initial diagnosis and treatment. They may need to refer her on if they cannot hospitalize her but they should perform the preliminary workup and diagnostic testing since this sounds to be serious.

What you're describing may be some type of infectious arthridite caused by either a tick born entity which will need antibiotic (maybe even IV) treatment or some other type of joint infestation by fungal or viral elements (less likely because of the distribution of being on both sides at once as well as the description of the lesions). If there are pictures of the blisters could you upload them to this website as .jpg files?

If this is PEMPHIGUS then, there are a couple of types. Pain is characteristic of this blistering disorder which if located in the mouth also cause extreme difficulty in chewing, eating, and swallowing. It is an autoimmune condition and presents with watery blisters over the skin in different parts of the body. Here is a link to an article that talks about this disorder and has pictures. Perhaps, her rash looks like this?

Has your neurologist ruled out every type of blood chemistry etc. that could explain what's been going on? If not, then, I would ask the PMD to pull your files and see which other lab tests could be done to improve the content of neuropathic lesions.
