What Causes Severe Lower Abdomen Pain With Pinkish Foam From Rectum After Appendectomy?

Question: Hello. I had an emergency appendectomy 3 weeks ago. This Saturday around 3 am I started experiencing what I think was very severe lower abdomen gas pains with pinkish foam from rectum. I have been on clear liquids since sat morning as I really have not been very hungry since surgery. Since last night every time I urinate I express clear foam with specks of blood from rectum. Should I be alarmed? See the surgeon? Thank you so much.

Hello Dr. XXXXXXX
I do not have history of bleeding, or constipation but do have a little hemorrhoid, some digestive issues since my gall bladder was removed in 1984, no vomiting and the pain is dull and takes up most of my middle to lower gut.

Thanks for writing back,

You had appendectomy 20 days back and since then you have had no issues , it means that it went well and it didn't get any complication. THere is 99% chance that the appendix would have got healed by now, as you are not having any abdominal distention and constipation it means that your gut is patent and there is not adhesion or blockage.


Now we come to the reason of pain and blood

There can be the following reasons of pain and blood:

1) Stomach or gut ulcer:

Due to use of painkillers after operation or just because you were predisposed they cause both pain and bleed but usually stomach bleed causes dark stools and it isn't fresh.

2) Gut infection:

Usually amoebic dysentary causes pain and blood in faeces but it usually causes mal formed stools which you didn't complain of.

3) It might be simple gut irritation:

this can settle with fibre and simple food intake

4) Haemorrhoids:

They can cause haemorrhage but lower gut pain suggests some issues in gut.


suggestions:

1)Don't panic

2) Don;t stay on clear liquids take simple soft foods plus fibre foods like whole wheat.

3) Good amount of water.

4) Take an antacid if there is stomach burn.

5) You can discuss with your doctor about hemorhoids and infection being a possibility.

6) Your doctor might wanna give you metronidazole 500mg thrice a day and a smooth muscle spasmolytic like mebeverine.

7) Keep a close look on your stools and see if bleed is decreasing increasing or staying same.


Things are expected to get better soon.
