Brief Answer:
Stool examination and ultrasound abdomen required.

Detailed Answer:
Hello,
Thanks for posting your query on Healthcaremagic.

On the basis of your history of on and off diarrhea, abdominal pain radiating to left side and episodic fever; I suggest you few investigations namely,
1. Stool examination -routine and for occult blood,
2. Ultrasound abdomen.

Stool examination especially during episodes of diarrhea will help to determine the nature of stools and most probable type of diarrhea.
Occult blood detects minute quantities of blood in stools which is not obvious to naked eye.
Blood in stools is seen in number of conditions like irritable bowel disease, inflammatory bowel diseases, polyp, malignancy etc.
You can take medication for symptomatic relief of diarrhea and abdominal pain.
Associated fever may be due to infectious diarrhea.

Ultrasound abdomen is required so that all abdominal organs can be assessed.
Abdominal pain can be due to pathology of abdominal organs.

You can post the reports of the above mentioned tests for further work up.

Thanks and take care
