What Causes Severe Abdominal Pain And Burning Sensation While Treating Gastritis?

Question: Good evening doctor. I would appreciate your opinion.I have had upper abdominal burning greater than four months.I have had endo by 2. The first showed bile gastritis resolving. Duo and esophagus normal. Pain continued for two months.I saw a professor at U of P.MRI normal Second endo normal with no gastritis. Doctor,I am still suffering.Prilosec 20 years. Said I should dc. I did and went to zantac. Still suffering.Prof said I have nothing treatable. 63 yo retire and frustrated.Non smoker and regular exercise.LFT XXXXXXX normal. I would appreciate an opinion and direction. Thank you.Sincerely XXXXXXX

Good evening.
Thanks for your query.
To recapitulate: Male/63 - non smoker - regular exercises - three beers a week and one coffee daily -burning may increase with large amount of food but can occur without. visited Doctors - took Prilosec for 20 years and now on Zantac - upper abdominal burning more than 4 months - First Endo showed resolving bile gastritis - pain continued for two months - second one showed normal - still suffering - Prof said nothing treatable - retired and frustrated ....

The commonest reasons we can think in view of burning upper abdominal pain in spite of Prilosex and Zantac are:
- Stress and/or anxiety play a very major role. This instigates extra acid secretions, reflux of bile to stomach and acid into the esophagus momentarily and may not be seen in the upper GI Endoscopy.
- Allergic phenomenon to scents, fumes, food additives, beverages or anything that can be found only by you and your family by keen observations and noting down the factors.
- Rare tumors in pancreas like gastrinoma which can be found on ultrasound and CT scan of the abdomen.
- Neurological problem like slipped disc at thoracic level causing some neuropathy, compression or so and causing referred pain in the upper abdomen.

I would advise in such a situation the following:
- Add on antacid liquid with anesthetic property like Mucaine gel or Happicid gel and to be started taking the moment with you get up along with Zantac Continue taking the gel every 3 hours particularly when you are on empty stomach.
- Small frequent meals. Non oily and non-spicy.
Minimum 6 to 8 hours sound sleep.
- To stop all foods, beverages and such things in life that either instigate your problems or enhance it.
- Motility regulator like Domperidone under the prescription of your Doctor.
Medications for stress and/or anxiety if required.
- You may please stop beer and coffee for some time and see if this improves the results.
