What Causes Sensitive Perineum When Diagnosed With Lichen Sclerosus?

Question: HI Doctor, I have a Lichen Sclerosus. Ive been dealing with my cuts at the bottom of vagina for years but after I stoped taking birth control pills, I don't cut there anymore. Unfortunately, my perineum is now more sensitive and is very easily to break the skin there. BUT FOR THE FIRST TIME IT GOT SWOLLEN, IT FEELS LIKE ITCH AND IT IS KEEP ON BREAKING AND MAKING NEW CUTS CONSTANTLY. How can I heal it? Because of my lichen condition I cant use steroid cremes or etc. Please help, I am a dancer and I need to fix this problem asap. THANK YOU!

Can you please provide some more information, so I can answer your question more accurately.
-- For how long have you had these cuts? Are they are of the lichen sclerosus, or made physically or you have any other possible cause of them?
-- Can you upload a picture so I can have a look at them? I need to rule out if they are infected, or fungal infection, or part of the lichen problem.
-- Why do you say you cannot use steroids, when the treatment of Lichen Schlerosus is with topical steroids? Do you have any other from of Lichen?
-- Have you used anything in the cats so far? Anything that you have noticed is helpful, or that makes it worse?

Each of these questions will help me understand more and so I can give you a better and helpful advise.

Hi Dr. Papaqako!

Thank you for you help.

I have information (swollen perineum) for 3 full days now.

Because some of the creams with steroids made my skin even thinner two of my doctors said that my skin is too sensitive to take more on my skin. From September on after I stopped my birth control pills I have many yeast infections and the only way I get healed is by taking a pills, not creams because they damage my vulva so much.

Since September I noticed that my perineum would cut a lot after I went to the bathroom when I swiped it. But now it got inflamed I think after sex and this never happened to me before. Today I noticed that my vulva is red on the inside, it got infected.

I didn't use anything on perineum yet.

I do have one picture, I don't know how to send it.

Hello dear N,

Thank you for the follow-up question and the extra information.

I looked carefully at the latest evidence of possible treatments of Lichen sclerosus when using topical steroids is not an option.
For women with anogenital lichen sclerosus there are alternative treatments like injectable corticosteroids such as triamcinolone or dexamethasone.
Also, systemic therapies may be considered when cases are refractory to first-line therapy but evidence is limited.
Other treatments such as cryotherapy, phototherapy, ultraviolet, and focused ultrasound have been tried but have very limited evidence.
For complications such as stenosis due to scarring or adhesions that inhibit urinary or sexual functioning, or for treatment of coexisting carcinoma can be tried surgery, but those are not your problem now.

I don't know how much this is helpful. But my recommendation is for you to talk to your local doctor (who can examine you also, and rule out the posibility of overlaping bacterial infection and need for antibiotics) about possiblities of getting one of these treatments there.
