Brief Answer:
MRI scan needed.

Detailed Answer:
Hello,
Thanks for posting your query.
The feeling of clicking or grinding with unstable shoulder joint is suggestive of a bone injury or a ligament tear. This can be properly diagnosed on an MRI scan of the shoulder as it is the best test to pick up any soft tissue injuries like ligament tears.
The presence of lump in the place of old injury is likely to be due to bone remodeling that takes may months to happen after healing. It needs a direct examination to confirm the diagnosis.
I hope this answers your query.

What about the cramping in the hand and why would the collarbone be sitting higher and more visible at the sternum? would this need surgery to fix

Brief Answer:
Cramping can be due to nerve root compression.

Detailed Answer:
Hello.
Thanks for writing again.
The cramping in the hand can be due to a mild nerve root compression at the level of cervical vertebra. The higher level of collar bone should become normal after remodeling. Usually it does not require a surgery but a direct examination by orthopedician will help in confirmation.
