What Causes Scrotal Pain With Hives After Consuming Cheese?

Question: My son feels like he was kicked in the balls. But was not. This pain has been a dull ache for over a week now. He has been checked to rule out appendix problems. Doctor stated he was low on potassium so gave him a drink - while he was in the ER. After blood work and a cat scan they sent him home. No diagnosis for why he was in pain. Still very uncomfortable and can't walk straight up and complained this morning of his right ear feeling strange like it was full and then he got dizzy. Any suggestions? He is a 21 year old male. Unfortunately, we do not have the results of test done on Friday. We now understand he has a portal at local health facility. We hope to get into that and obtain results of anything they ordered and did to him when he was in the ER on Friday. I feel they are missing something.
He has also experienced major hive breakouts for several days at a time over the past year or so. We can never find out the reason for this - Doctor chalked it up to nerves perhaps? He has recently discovered he has a problem eating cheese and milk products so quite frequently will take special pills at Costco which will allow him to eat certain dairy products. He must take these pills several at a time just prior to eating such foods. He general stays away from them. But every now and then wants to give in.

1) When did the pain begin: He just woke up one morning about 1 week ago and had the pain. And it hasn't gone away since. Not better and not worse. Just a nagging dull ache in his lower pelvic area. Right Quadrant side. And he explained his feeling as getting kicked in his nuts and the pain is still there. He went on to explain. Not the first sensation you get at impact, but the after math below the belly button area in lower front middle pelvic area. The pain he feels is a slow wave of pain which comes more intense and then lightens up continuing throughout the day. He can't seem to walk fast and stand up straight as the discomfort is too bad. Standing up straight is ok, but must walk slow or he gets a more intense aching sensation and needs to bend over. The pain does not intensify when turning over in bed or twisting side to side.
2) Any flu like symptoms: No symptoms of flu - such as chills, fever, throwing up, runny nose or achy all over.
3) Any urinary problems: No Urinary complaints - No burning sensation or discoloring of urine. Color is his normal color, lighter yellow shade. He does have a problem starting to urinate. He has complained about experiencing this for about the past year off and on.
4) Any fever ?: Had very slight fever a few days ago. But I feel more because he was so upset and worked himself up. Doesn't seem to have any low grade ongoing or high grade fever since pain started. Temp was normal at 98 degrees the day he was in the ER. - Friday, Oct. 28th, 2015.
5) When was his last attack of hives: His last hive attack was about a month or so ago (August) and lasted on and off for a few weeks. This made him very stressed as nothing would make them go away. Oatmeal baths, anti-itch creams. He did take Benadryl constantly and found that when he would first take it - they would get bearable and sort of go away, but within about two hours or so when it wore off, the hives would come back full blows. The hives appeared more on his extremities rather than the trunk of his body. We even tried changing laundry soaps. He did have 3 similar hives situations in the past couple of years off and on. First one started during his last year of high school. And again this past February and then again in August this past summer. ( He graduated high school in 2013)
6) Any vomiing or nausea? Yes, he has been having some mild nausea on and off and feeling like he wanted to vomit (but never actually vomited) . No dry heaving or forceful vomiting. He actually just told me he doesn't really feel nauseas - just did a few times on Thursday and Friday - 2 days ago. However, this morning his ear felt like it had fluid in it and it filled up and he felt very dizzy and that dizzy sensation made him feel a little nauseous. This situation with is ear happened a couple of times today. He hasn't felt that before in his head.
7) When did he first start having sensitivity to dairy, cheese, products? He started having reactions to milk, ice cream and anything dairy about 5 years ago. He would get a terrible stomach ache and sit in the bathroom forever. He told me his stomach hurt and had bad cramps and diarrhea. He started to see a pattern when he had ice cream, a glass of milk or even a grilled cheese sandwich. His brother's girlfriend has a medical issue (can't remember the name of the condition) and takes Lactose pills to help with her reaction to diary. So XXXXXXX out of experimenting with his issues tried some one day to see if they would help him and now uses them all the time. He takes them everywhere he goes - even keeps a bottle of them in his car. He commented that these pills are starting not to work for him as well as they have in the past.
8) We will try to dig up any blood work or reports/results from scans, ultra sounds, etc... I'll have to hunt them down and see how I acquire them from his doctors.
9) He mentioned something to me about the doctor stated he was low on Potassium and so they gave him a supplement of potassium to drink during his ER visit last Friday.

Thanks for the detailed updates and your reports.Yours is a very complex and intricate case,which is getting complicated with food allergy, with highly sensitive Auto-immune reactive System,which would explain the complaints which you suffer from lately.

Explanation/Reasoning- of Your Complaints-
I reviewed your updates in context with the Dull nagging Acute on Chronic Abdominal Pain,which is not yet relieved with medicines-
Upon reviewing the whole details of the abdominal pain with nausea- and the ear itching-In My opinion-
-The feeling of the right lower quadrant pain is due to the Mesenteric Adenitis- from the 5 yr old –Cheese and Milk Allergy –with Lactose intolerance.
-Occasional hives attacks are indicative of –Cheese Allergy/ie.allergy to Casein Protein.
As submitted from You, the sequence starts with ice cream, glass of milk, and dairy products,which proves that You have this basic problem of Chees/Milk/Dairy Product Allergy,with histaminic reactions,which explains all your complaints in short.
Hypokalemia –seems to be due to the chronic enterocolitis- with Milk / dairy product Allergy-allergy to casein protein and impaired nutrition with this chronic problem.
Regular intake of the lactase enzyme pills has not made any change with all these complaints,in your case as submitted by you.This indicates your case is not of-Lactose Intolerance(in which lactase enzyme deficiencey in the colon is found)

- about Your Screwing abdominal Pain/or feeling of kicked in the balls-
Cause of this right lower abdominal pain is due to the Mesenteric Adenitis and Mesenteric Inflammation,which pinpoints the pain to right lower quadrant of abdomen. XXXXXXX seems to suffer from-cheese /Dairy product Allergy and all his complaints could be avoided by ONLY TOTAL AVOIDANCE OF MILK/CHEESE /DAIRY PRODUCTS lately over last 5 yrs period,which is the root cause of all these complaints lately faced by YOU.

ALERTS / And Life-Diet Changes-
Hence You need to be very cautious with Cheese/Milk dairy products intake-and should avoid it as far as possible. Feverish feel is due to the enterocolitis-
