Question: Hi Doctor,

I had a baby girl who is now 2 weeks and 3 days old. she was born at 39 weeks normal birth. her birth weight was 2.5kg and now she is 3.1kg, it was otherwise normal birth, at her birth she had some mucus on nose which was relieved by nasal spray given by nursery during our stay at hospital

about yesterday, again, i felt like she had a runny nose and i could here some mucus when she was breathing.
she also had rash (pimples with redness and white tips) around her eyes and forehead.
my wife took her to hospital today as i couldn't go with her due to busy work.
My wife called me and she said the doctor is suggesting to keep in Hospital for 2 days to observe her. so i called the Doctor and spoke to him to get more details of what's going on.
The doctor told me that the baby is normal, she doesn't have fever or any signs of infection such as sleepiness, blue lips or anything to worry about, however he wants her to stay at hospital for 2 days to observe her and avoid any possibility of developing bronchiolitis or anything else. he also said he will use nebulizer and nasal spray to help her breath better.
the problem is we are not at home country and hospital stay is very expensive so i asked him if it's possible if we buy the nebulizer and the medications and we do everything as per your prescription.. he said there is totally no problem with that and to bring her to him in 2 days.

So we agreed on that.
i will go to buy a nebulizer and i will inform you the dosage he prescribed and the medications.
My worry is that i don't know how to use a nebulizer and i'm hoping to use it well.


1.do you think it's ok to bring her home or you suggest it's better if she stays at home?
2.we will buy omron nebulizer, is it ok to use that for infant?
3.once i get it i will let you know about all medications and you can suggest me how to give the nebulizer and how to use the medicines?

I have gone carefully through the details and understand your concern. Most often such symptoms are due to viral infections and antibiotics are ineffective in such cases. Home nebulization can indeed be useful in such occasions. It is absolutely ok to be used in case of an infant.

About the techniques of using a nebulizer, a demonstration would be needed. You can look up on youtube for the same. Alternatively you can get a demonstration from your doctor. If you want, I can definitely clarify your queries.

Hi Doctor,

thanks for the quick response.

I started administering her the nebulizer since last night every 4 hours as per doctors prescription . they are Ventolin,saline and other medication mixed and put together to finish in 15 minutes.

I also give her nasal spray every 4 hours.

it's been 24 hours since the administration of the nebulizers and nasal spray, however, I still don't see much improvement in terms of breathing.
I can see flaring on her nose and sometimes she is coughing and crying. she also has difficulty nursing milk from the mother.
The doctor called us today and he said based on the results of a test they did where they took a sample of her nasal mucus they saw it was RSV virus, he told her to keep watching her carefully.

her mother told me that she saw her once turn green on her lips and near eyes for few seconds while she was trying to cough and then got better.

Now she is breathing but still with difficulty she fed a little bit as well.

what do you suggest us to do Doctor?

i wanted to ask you, i saw some videos in youtube where they do something called nasal irrigation whereby they fill a normal nasal aspirator with water and then they push the water in one nose while holding baby forward and allow the mucus to come out from the other nostril. Do you think i can do that for a 2 weeks old and given her current condition? do you suggest to do it?

also if there is other thing you suggest me to do to help release the mucus that's making it so difficult for her to breath?

Thanks for writing back. sorry for not being able to respond faster.

Please note that such infections take time to recover. You should not expect response within a day or two. it will take 5 to 7 days and you would need to be patient. It is unnecessary to repeatedly gave the nasal spray unless nasal block is suspected. Continue the nebulization you will gradually see improvement. I do not think that you need to rush to the doctor right now.

Legal education as a different purpose and is not helping the situation. I would not suggest at the moment the nebulization will help in reducing the mucus. be patient.

final follow-up question.

my daughter is doing very well now thank God. it's like you said we need some time

currently she takes
1.0.9% physiologic saline
2..ventoline 2.5mg/2.5ml (she takes 0.4ml every 6 hours )
3.atrovent 250mcg/2ml (she takes 0.4ml which means 50mcg every 6 hours)..

the physiologic saline is 0.9%/5ml
The way the nurse told me to give her is add physiologic saline to Ventolin every three hours and physiologic saline and Atrovent the next three hours and so on that way there won't be a big gap between the two medicines and we achieve the 6 hours for each medicine

I was giving her the medication as prescribed, however now I'm little worried because I was giving her physiologic saline 5ml (which means the whole bottle) while when I read the prescription now it only says 1ml.
the problem is in the prescription it doesn't clarify if it's 1ml it only says (1.00) which to me I thought it meant 1ml. when I checked carefully and called the doctor he told me it meant only 1ml and not the whole bottle (the whole bottle is 0.9%/5ml) . you can see how it's written in the prescription paper I uploaded above.

1. is that considered overdosing because since I was mixing physiologic solution every time I gave her Atrovent or Ventolin it means I was giving her overdosage of 5ml instead 1ml every 3 hours for almost 5 days now, would that cause a problem for her lungs should I go to emergency ? is that considered serious overdosage? or it's acceptable?

Let me assure you that you can feel free to give nebulization with normal saline or physiological saline as many times as you want and it will never count as an overdose. Although it is not essential to add saline to the medicines, it would be absolutely fine if you do and would not be a cause for worry. The medicines are proper and feel free to continue them as directed.

The physiologic saline basically acts similar to water and helps in hydrating the respiratory tracts and thus helps in diluting the mucus thereby providing relief.
