Question: I have taken alpha blocker for one month and getting retrograde ejaculation. Now alpha blocker is stopped. After how long ejaculation will become normal.

Alpha blockers usually are short acting and act immediately usually with in 24 to 48 hours. Hence on stopping it the side effects like stuffiness of nose and headaches usually disappear almost immediately. But some side effects like ejaculation problems may persist for about a week.

Then pl advise if I should continue taking alpha blocker as this was given as I had stones in both kidneys and ureters where the left side stones are of 5 mm sizes which have now been removed by URS and RIRS. The right side stones are small like 2.80 mm sizes and maybe they are still there as I hv not seen them coming out. I am taking alkasol for last one month twice daily and I don't have any discomfort so far on my right side. What can I do to possibly dissolve the small ones on the right side. Does taking Kulthi water soaked overnight, in the morning, help ?

Alpha blockers only work for stones in middle and lower part of ureter. They relax the tubes draining from the kidneys(called ureter). So stones come down quicker and with less pain. It does not help the stones in the kidney. Small stones in the kidney also do not cause any pain but causes pain when drops down to ureter.

If the 2.8 mm stones on the right side was in the ureter on previous scan then I would advice you to get an ultrasound to see whether the stone has passed. If it was in the kidney then you can stop the alpha blocker straight away(an ultrasound may not pick up such a small stone in the kidney -so no use in repeating the ultrasound)

Regarding the preventive method for stones and to prevent the right side stones from increasing- please drink adequate fluids to make urine colorless, reduce salt in diet and avoid red meat and reduce animal protein.

You can continue taking alkasol for 2-3 months and stop as most people find it difficult to continue it for longer periods. Instead you can drink freshly squeezed lemon juice every day. Kulthi water helps by adding to fluid intake but I do not believe it works in any other way.

Thx for yr reply. Pl inform if USG will be able to detect small stones when they come into the ureter. Also how does taking Alkasol help if I take it for 2 or 3 months . Reg taking lemon juice shall I take one whole lemon juice ?

If small stones drop into ureter it is likely to show what is called hydronephrosis or prominence of pelvicalyceal system of kidney. This associated with pain indicates that there is stone in ureter. Only very rarely there will not be hydronephrosis but there will be pain prompting us to go for CT scan if ultrasound is normal.

Citrate present in alkasol is helpful in preventing formation of stone or enlargement of stones. Lemon has very high concentration of citrate. Two lemon per day is equivalent to taking alkasol three times per day which is the normal recommended dose. Alkasol also has sodium which may be undesirable if taken for prolonged periods.

Thx for yr reply. Now pl tell me what painkiller can I take and what is the dose when there is pain in the kidney or ureter due to stone. The medicine should be fast acting ie in abt 30 mins pain should stop. How frequently can I take it. Pl name brands which I can tell medicine shops and not composition.

I normally prescribe my patients a tablet called Naproxen and the dose depends on severity of pain - from 250 mg to 500 mg twice daily provided you do not have ulcer, bleeding tendencies or kidney failure or asthma. You will have to get the prescription from your doctor.

Will Spasmoproxyvon also work ? Or whether Naproxen is better. How long it takes to work. Earlier I had taken Spasmoproxyvon but the pain did not reduce and then Voveron injection had to be given.

I have got the stone analysed. It is Calcium Oxalate Monohydrate 90% and Calcium Oxalate Dihydrate 10%. Pl inform what all I have to do to avoid recurrence of stone.

To avoid recurrence of stones
1) Increase fluid intake enough to make the urine colourless.
2)Decrease salt in your diet
3) If you are non vegetarian, stop taking red meat and decrease animal protein- chicken, fish egg etc.
4) Increase citrate in diet - two lemons in a day in the form of lemon juice.
5) Decrease Oxalate in your diet - reduce nuts, chocolates, spinach and other leafy vegetables, radish, tomatoes, strawberry.
6)Increase diary products like milk, cheese, yogurt.
