Brief Answer:
May be balanoposthitis and dermatitis

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.

This looks to be a problem called balanoposthitis and associated dermatitis of the scrotum ( testicles ).

The effective treatment is as follows:
Clean the area with plain water 2 to 4 times a day and and keep it dry with a pure cotton cloth or a dryer.
you may apply liquid Povidone Iodine locally or a Metronidazole cream- nothing else at all. We have seen no results with any other solution, cream or ointment. So do not try anything else; if you are using any, please stop it.

A course of antibiotic ( you are already under a good antibiotic) and metronidazole orally for 5 to 7 days and a single dose of oral anti-fungal tablet should help you to get a cure.
Please get your Blood Sugar checked.

If you are sexually active , your partner too needs to take the same treatment.

I hope this answer helps you to get a proper solution, please feel free to ask further if you need any additional information and you may rate this answer before closing.

Wishing you an early and complete recovery....

How often should apply the cream topically, and a single dose of oral anti fungal cream.. Is that different than metronadazole oral treatment? I am searching and seems i can get all this over the counter?

Brief Answer:
Thanks for your feedback

Detailed Answer:
Liquid Povidone Iodine locally or a Metronidazole cream- can be applied 2 to 4 times daily depending upon your working hours and suitability.

Single dose is of anti-fungal oral tablet like Fluconazole. It is different from Metronidazole oral tablets.

If available over the counter - well, otherwise you may need a prescription of a local Doctor.

Thanks doc.. Is burning when urinating and pee pee dribbling out afterassociated with this? It seems to be worse at times than others almost like a flare up. I had the encounter in april and marks didnt show for about a month and a half two months after. I was prescribed a topical ointment for teast infection at first then recommended to see a urologist where he prescribed the other medications.. Also why should i get my blood sugar checked?

Brief Answer:
all things can be associated.

Detailed Answer:
Hi.
Yes, burning urination and dribbling can be associated with this.
Diabetes can be a reason for not healing early or completely. Balano-posthitis can be the first presentation in a few patients, hence it is better to rule out diabetes , hence get the blood sugar checked.

Noted the history given above. There can be latent period of having an encounter and actually have symptoms. This can be one and half months as seen in your case.
