I have gone through your query and i have also reviewed the image.
This seems like a irritant dermatitis/ diaper dermatitis Or it could be a insect bite.
Do you use a diaper for baby? and Is this area covered by a diaper?
He can get dry/irritated skin during the winter- we use aquaphor all over to help with that. We also use Aveeno soap & free/clear detergent. This started towards the end of the week while he was staying at a relatives house, but I think he's had a similar breakout once before, to a lesser degree.
Topical steroid+antibacterial oint; treat for scabies as well
