Brief Answer:
Bed rest and injectable progesterone.

Detailed Answer:
Hello Mr XXXXXXX thanks for trusting health care magic.
The medicines prescribed to your wife is appropriate and in adequate dosage.
But ever since she has started with slight brownish discharge with history of clots in ultrasound she should be put on -
- injection susten daily and stop microgest till injectable progesterone is administered daily.
- stop loprin
- complete bed rest
- no intercourse
- no constipation
- light food with lots of fruits and salad.
Follow this for 5 days and then get an ultrasound done and if everything goes well then routine scheduled visit to gynecologist.
But in case she starts bleeding fresh blood or ultrasound has evidence of clot then visit ER as soon as possible.
I wish her the best of health.
Take care.
