What Causes Recurrent Ulceration Over The Tonsil Area?

Question: Hi Doctor. I'm a 26 yr old male in good health. I have always had severe sinus infections my whole life, have had to see many ENT doctors, and eventually had my tonsils removed when I was 18. Over the past 2 years I've noticed I get a pretty bad sinus infection two to three times a year. There always seems to be an ulcer that forms on the left side of the back of my throat, above it near the uvula, or right next to where my left tonsil used to be. The last sinus infection I had, had the ulcer right near where my left tonsil used to be. This was treated with a series of antibiotics in November 2013. Prior to that in March 2013, I had a similar experience that was treated. On Monday, April 28th I've developed an ulcer once again on the left side of the soft palate of my mouth near the uvula. HOWEVER, I have no fever, no sinus infection, no post nasal drip, etc. It's an oval sore, grey-yellow in color, red all around it, and of course is making my left ear ache. I've been taking extra strength tylenol twice a day since monday to relieve the pain. Thank you for your help.

Greetings of the day

Dear XXXX

Considering your young age and absence of such Ulceration over other areas it's unlikely to be serious.
I could consider the following two probability as the cause of recurrent Ulceration over the tonsil area
1. Recurrent apthous ulcer
2. Oral candidiasis: much more likely in view of surrounding angry looking red area

I would suggest you the following which will definitely help

1. Oral Multivitamin supplements
2. Warm saline gargling : To a glass of lukewarm water add 2 spoons of common table salt. Gargle throat with it thrice a day
3. Chlorhexidine gargle : Alternate it with saline gargling
4. Antifungal lozenges : Nystatin lozenges twice a day for a week will be helpful.

If the lesion still persists then it will require punch biopsy for further evaluation.
