What Causes Rectal Bleeding With Severe Abdominal Pain?

Question: I had started to notice rectal bleeding last winter. I was under a lot of stress. i finally had a colonoscopy done sept 2015 the doc didn't find any polips but did find a couple spots in my lower colon, they are going to test for IBD for ulcerative colitis or chrons. I haven't had loose runny stools in the last 5-7 years but when i was younger i would experience extreme pain to the point of almost passing out while having bowel movements. I've been treated for ulcers since i was 15, or 17, have had extreme abdominal pain without bowel movement at times in my mid 20's so painful I couldn't get off the floor, or talk or ask for help.. After I had my first dayghter, age 25, I couldn't eat unless someone came to watch the baby due to the abdominal pain that followed eating. Sugar and rich foods set it off.. I havent had problems with loose stools since I injured my 2 lower vertebreas and have been on a pain management plan since. I know the ms contin has a gi slowing effect and i make sure to walk, drink lots of water, and eak raw vegies so I won't get a block. In the last 4 years my neck has been having spasams when i swallow, sometimes when i don't, they just happen. I can breathe fine so there is no constriction in my airway. I have an endoscope scheduled for march 2016 because I'm finishing a college degree then will be studying and taking a State Board Exam and the anesthesia caused me to loose any memory of reading my courses, taking notes, or even completing assignments. I didn't remember any of what I'd started studying in August, prior to the colonoscopy, so, i'll wait until I'm done studying and testing before I go through thaat again. What I was wondering was: I really didn't tell anyone when i was little about passing out while having watery stools, for having to lay on the side of the sink to keep from falling out from time to time. Could that abdominal history, plus ulcers be related to this? I just thought everyone hurt sometimes like that. also, my drinking turned into daily blackouts in 2002 after some extremely traumatic events in my life. I drank daily, all day, every day from the time I could find any left over from the night before until i passed out at night. Thankfully it only lasted 10 months, before that I was a Good mom, wife, and nurse for many years, drank on weekends sometimes. I haven't drank since 2002, haven't nursed since then either, but i was raised in a home where the kids drank beer when we went fishing and at our parents partts. I started drinking maybe age 5-7. not daily but did drink. We lived in the north, with the local pubs where 13 year olds could drink in the bar and drinking beer and driving was the norm. Like is said it was those 10 months that flipped my alcoholic switch but .... I also think the lining of the intestine grows new cells. anyway does it sould like my pain managment medication has masked my symptoms. oh yeah. I've been small all my life, I forget to eat. and do stay away from foods that hurt after i eat them.fruit hurt me a lot. my paternal grandma died at 89 years old with colon cancer a
that mats. to her liver. I don't take have her build, i'm slim, lean and muscular, well at least until last year when stress and depression hit bad. I have clinical depression and work with medication as well as postitive psychology, ie learning to reduce stress.oh yeah my pain management is controlling my pain along with walking. I have pain but I can function and have good quality of life now. I've been on the same dose for 5 years now. You never get pain free, you get to be able to walk and stand and work and smile , anyway, this is an issue with my sister who thinks i need surgery on my back. i'm not ready for that at all

At the outset, let me apologize for the late reply. I have been answering on this platform for more then 3 years and I have never seen a query being unanswered for this long.

Now coming back to your post, though it was a lengthy I read through it with diligence and from what I understand you have had multiple episodes of passing out while trying to pass stools and have been had to deal with painful bowel movements. You have had colonoscopy few months ago where some abnormal colon areas were found.

In this context unless the region is studied further I would not be able to comment on that spots. It may be responsible for this painful episode. If subsequent testings in future (after your course is completed) doesn't reveal fruitful results, I might suggest that you discuss with the spine surgeon whether or not spinal nerve injuries could be contributing to this discomfort. Lower vertebral nerve injuries sometimes leads to autonomic disturbances which are painful.

About the vasovagal syncope which I referred to in my brief answer, in some people an act of defection especially a painful one leads to drop in blood pressure which causes passing out spells. Your description of those events indicates towards syncopal attacks. I would ask your doctors to check blood pressures and autonomic functions.

Besides these, yes, those pain medications especially opioids may adding to slow bowel movements. I understand you require those. But as bowel movements are affected discuss about starting alternative pain control strategies such as tricyclic antidepressants or even atypical antiepileptic drugs. These may lower your pain threshold and thus reducing the need for opioids.

I have tried to address all your concerns; but if you feel I have missed a few details / concerns please let me know.

I've tried those medications,elevil and gabopantin, neither worked enough to continue through the side effects. They sedated me to much. As far as pain management, the medicine I'm at now allow me to live my life without the "hangover" effects of those others. As long as my pain levels stay where they are now I will leave that alone. Thank you for your help.

Try antispasmodic to relieve discomfort....

Detailed Answer:
I empathize with you. You may continue with the pain medication that you are on now until the surgeon plans a different treatment. In this circumstance, you may have to wait until the next gastro appointment and hopefully the scope plus biopsy results becomes helpful. Until then I would suggest using antispasmodic agents such as dicyclomine. It could help in relieving the painful bowel complaints.

In addition, I would also suggest that you use a stool softener along with fibre rich diet.

Lastly, you should consider reviewing with your spine surgeon / neurosurgeon in case gastroenterologist have no answers to this problem.
