I have clinical depression and work with medication as well as postitive psychology, ie learning to reduce stress.oh yeah my pain management is controlling my pain along with walking. I have pain but I can function and have good quality of life now. I've been on the same dose for 5 years now. At the outset, let me apologize for the late reply. I have been answering on this platform for more then 3 years and I have never seen a query being unanswered for this long.
Now coming back to your post, though it was a lengthy I read through it with diligence and from what I understand you have had multiple episodes of passing out while trying to pass stools and have been had to deal with painful bowel movements. You have had colonoscopy few months ago where some abnormal colon areas were found.
In this context unless the region is studied further I would not be able to comment on that spots. It may be responsible for this painful episode. If subsequent testings in future (after your course is completed) doesn't reveal fruitful results, I might suggest that you discuss with the spine surgeon whether or not spinal nerve injuries could be contributing to this discomfort. Lower vertebral nerve injuries sometimes leads to autonomic disturbances which are painful.
About the vasovagal syncope which I referred to in my brief answer, in some people an act of defection especially a painful one leads to drop in blood pressure which causes passing out spells. Your description of those events indicates towards syncopal attacks. I would ask your doctors to check blood pressures and autonomic functions.
Besides these, yes, those pain medications especially opioids may adding to slow bowel movements. I understand you require those. But as bowel movements are affected discuss about starting alternative pain control strategies such as tricyclic antidepressants or even atypical antiepileptic drugs. These may lower your pain threshold and thus reducing the need for opioids.

I empathize with you. You may continue with the pain medication that you are on now until the surgeon plans a different treatment. In this circumstance, you may have to wait until the next gastro appointment and hopefully the scope plus biopsy results becomes helpful. Until then I would suggest using antispasmodic agents such as dicyclomine. It could help in relieving the painful bowel complaints.
In addition, I would also suggest that you use a stool softener along with fibre rich diet.
Lastly, you should consider reviewing with your spine surgeon / neurosurgeon in case gastroenterologist have no answers to this problem.
