What Causes Pressure Sensation On Temple Area When On Diltiazem?

Question: I was put on 120 mg of diltiazem twice a day for a total of 240 mg after 2 close SVT/maybe some AFIb.
It does not make feel that good but I am doing okay on it. I have been on it Tuesday the 9th in Er. The day after I took 120 on Wednesday and Thursday. I took the full 240 yesterday after taking to the nurse at my EP office. Last night I experienced a numbing/pressure sensation in my checks and temple area. I still took the second dose after that. This morning it is there lighter than last night. Should I keep taking meds or should I wean off by going back to 120.

I understand your concern, and would like to explain that Diltiazem may exert adverse effects like headache or edema in more than 10% of patients, also myalgia in around 2-3% of patients.

Diltiazem may be associated also with dizziness, bradyarrhythmia, hypotension, flushing, dyspnea, constipation, increased blood ALT & AST, alkaline phosphatase, etc.

Your numbness/pressure sensation in your cheeks and temple area may be adverse effects of Diltiazem.

In such case, I would recommend to keep monitoring your ongoing complains, your blood pressure and heart rate (several times daily), as well as any other mentioned possible adverse effects.

Dosing of Diltiazem may vary from conventional 30 mg every 6 hours, to 120 mg q12 hours to 480 or 540 mg /day, depending on Diltiazem formulation (SR, XR, CD, XT, LA).

So, my advice is to return to the prior 120 mg/day, if symptoms (these or even other appearing adverse effects) persist to be disturbing and consult immediately with your prescribing doctor, for a safer profile alternative therapy.
