What Causes Persistent Sore Throat While Suffering From Viral Infection?

Question: hi,


I have a question regarding my 3.5 year old son. he came down with viral infection in January, although viral as he had persistent sore throat he was given antibiotic (cephlaxin) we gave him for 5 days and he become fine. the week after he recovered my in-laws came from XXXXXXX and they had fever when they arrived. son picked up infection again from them.

this time the fever continued (not high but mild) for a week, in between we saw the doctor who asked us to give him another course of the same antibiotics, but this time asked us to give him for 7 days. which we did and as I mentioned the fever persisted for 7 days.

The next day after finishing antibiotics, son suddenly started asking for water every few minutes, he would drink and in a little while would want to urinate. We waited for 3 days and took him to hospital, they checked his blood sugar and glucose level in urine and said it was completely normal and rule out diabetes. They did osmalality test and initially said his salt levels were low so wanted endocrinologist to check who in turn said it may not be insipidus either and discharged saying nothing to worry.

His water drinking habit seemed to slowly get back to normal and urination came to complete normal in a week's time.

One thing we did notice that when he wanted water we would stick his tongue out and make his face as if it hurts and then immediately ask for water.

2 days ago I used his oral b electric toothbrush which only week before his water episode started i had purchased and brushed his teeth with it, and I had stopped using that brush after the episode, but 2 days ago I used it again, now he is asking for water again, this time he only sips very small quantity everytime but asks very frequently. I can't visibly see any blisters though now he urinates although not frequently but more than normal.

Another thing since my in laws arrived is he seems to have disturbed sleep in the night, especially on the days he doesn't go to childcare and sleeps in the daytime at home, he is very disturbed at night. 

I have mentioned all the symptoms and as a parent really worried. I would like to know what is causing all this.

I have gone carefully through your query and understand your concerns. Let me remark that intracellular sodium depletion can manifest with increased thirst. It seems that the sodium levels in case of your child is somehow getting disturbed. Rather than giving plain water I would insist you to give him ORS (oral rehydration solution) since it contains some electrolytes alongside. Give it sip by sip and gradually till his thirst resolves. I hope that the outcomes would be much better.

Thanks for writing back. It not not something to be worried about but nevertheless, the situation requires a proper monitoring and follow up. Discuss with your doctor whether he thinks of Syndrome of inappropriate antidiuretic hormone secretion (SIADH) to be the possible condition. This is a treatable condition and needs treatment of the underlying cause. With treatment it should resolve soon.
