What Causes Persistent Low Grade Fever In A Child?

Question: My 10 yr old son had typhi dot igm test positive on 3rd november . He had been having fever around 99.1 since some days.He was prescribed cefo az 500 twice for 7 days. After the course, he still has fever(99 - 100), and a widal done yesterday cane negative.doc has made him do a bloodculture.Any suggestions as to what he has?

Typhidot is rapid diagnostic test for diagnosis Typhoid fever and becomes positive within 2 to 3 days following infection. WIDAL test usually becomes positive after a week of infection. Typhidot is more sensitive and specific test than WIDAL. Positive test denotes Typhoid.

In case of Typhoid fever the temperature should start to decrease within 2 to 3 days of starting antibiotics . Persistent fever could denote either of the following
1. Typhoid bacteria is resistant to antibiotic being prescribed
2. Alternate cause for fever : The cause for fever is other than Typhoid.

As the fever is still persisting even after use of appropriate antibiotics, it is important to do blood culture (it is more sensitive and specific for confirmation compared to WIDAL and Typhidot ). I would suggest you to wait culture report .
