What Causes Persistent Abscess In Tonsil Despite Taking Medication?

Question: My 18 yr old daughter was initially diagnosed with a possible abscess in one tonsil and prescribed clindamycin. In a follow up visit two days later, at which point she was improved but not as much as the doctor had thought she might be, she was diagnosed with mono. The doctor strongly recommends taking her off the clindamycin but conceded that there was a possibility she could have a secondary infection in her tonsils and that we could, if we really wanted to, leave her on the antibiotic for a 7 day course. Meanwhile, her throat feels greatly improved (which makes me think she needs the antibiotic) but I also have seen on-line that sometimes people get a extensive rash when sick with mono and are on antibiotics. Which is the safer course - to keep her on the clindamycin or take her off?

In my opinion, the clindamycin course should be completed. Stopping antibiotics midway can cause resistance to develop in the bugs. Resistance to antibiotics occurs in sub optimal dosing and incomplete therapy. The fact that no rash was developed in the initial phase virtually rules out the possibility that she may be allergic to the medication or could worsen the mono. This is strictly my opinion only based on my clinical experience. A better option would be to take a second opinion based on a comprehensive history and physical examination that visualizes any residual abscess.
