What Causes Persistent UTI Symptoms Even After Taking Augmentin?

Question: Hi

I have had UTI symptoms multiple times in last year. I had tested my urine as well. And would be OK after a few days. Twice I treated myself with just being on liquids and lots of water and was fine. But since December I have not been OK. I have been to my family doctor and got an ultrasound and urine test and nothing came up in December mid. I was fine for a few days and then in XXXXXXX post my periods the discomfort came back. I consulted a gynecologist and was prescribed 7 day vaginal tablets, uriliser and oxofloxcin. I was OK for another 2 weeks after that before the discomfort came back. I went to another gynecologist and tested urine again. This time urine test had pus cells but nothing came up in urine culture. Vaginal and Cervix were checked and clean. I was prescribed augmentin 3 times a day for 5 days. But a day after my last dose the symptoms are back. It's ruining both my personal and professional life as I am not able to go out and have to take leave often. I am unable to concentrate on my work. And am unable to plan a baby also. Please help.

Thanks for your query,based on the facts that you have posted it appears that you have Abacterial Noninfectious Cystitis.

This is condition where one has symptoms similar like symptoms of UTI but routine Urine test and Urine culture is always negative .This is mostly due to irritation of the bladder and it is always difficult to establish a cause of the symptoms .

You have to treat the symptoms by drugs and by bladder training .

I treat my patients of Abacterial Cystitis on following lines
1) Bladder training by emptying the bladder every one and half hour irrespective of whether you have urge to pass urine or not.Gradually increase this interval up to 3 hours
2) Anti Cholinergic medicines like Darifenacin or Solifenacin once daily .This helps to reduce urgency for urination .
3) Urinary antiseptic like Pyrideum or Nitrofurantoin twice daily this helps to reduce bladder pain.
4)Avoid liquids that irritate bladder like Coffee,alcohol,citrus fruit juices .
5)Pelvic floor muscle strengthening exercises like Kegel Exercise This helps to get relief from urgency.

To be honest this is embarrassing disease for patient as well as for the treating physician.

Thanks a lot doctor. Which specialist would you recommend for me to consult now in person? Or can you recommend the dose and medicines?

And also what could be the cause of it as it's only since last year XXXXXXX that I have these symptoms. I also experience high hair fall and dandruff. General feeling of being unwell and some headache and nausea
Rarely I also have some pain in abdomen and once I also had shooting pain in left aide of my abdomen. But that was only once.

Urine test and culture mid december was clean but
Urine test XXXXXXX end had pus cells 20-30, rbc 2-4, epithelial cells 6-8 while urine culture was clean.

Have taken oflo, pan 40, urilizer, cranbe, clvigo forte in XXXXXXX and then augmentin 625, urispass and citralka in Feb.

Should I go for a urine test again?

Thanks for follow up.
Absence of any positive evidence in all the investigations is the feature of this disease .

In between you get positive findings of suggestive of infection but it never respond to antibiotics .

Follow the guidelines I have suggested I am sure it will work for you.
