What Causes Penile Swelling In A Child Post Circumcision?

Question: Hi Doctor, I have a question regarding plastibell circumcision.
My son is 12 months old.. 7 days ago i did circumcision for him (06/09/2015). on 10/09 (5 days post circ) we went back to the doctor to double check on him as we felt that his penis was swollen and there was little more swelling underneath his penis (lower part of penis), the doctor checked and he said it looks normal and nothing worry about, he said that he is travelling for 2 weeks vacation so he will remove the upper part of plausible and just leave the ring.

when we got back home, in the morning i tried changing the nappy for my son and he cried a lot because i pulled the nappy while his tip of penis was stuck to the nappy. i cleaned his penis and there was little bleeding.
next mornig the same thing happened but little more bleeding this time and i quickly washed him and he got better and i put him in a salt water for a while( suggested by doctor)..

Now he is better as i started applying vaseline in the nappy to avoid sticking .

but i see a very weird swelling underneath his penis between shaft and lower part, and there is white/cheesy material outside it on the side. the ring got cut and some side of the penis has no ring and the rest is still encircled with ring.

he has no fever, or swelling in abdominal parts etc
1.what could this swelling be?
2.does it need an urgent check up

As an Urologist, let me assure you, there's nothing to worry about.
Having gone through the photos, it looks normal.
The swelling usually subsides in a weeks' time.
Avoid salt water washing of the penis.
Then use only the ointment, twice daily.
Avoid applying vaseline to the nappy. Just remove it gently.
There's no need for urgent check-up.
Wishing your son a speedy recovery.

Thanks for the quick response Doctor. he looked really good today, playing and acting normal. Thank you for the reassurance.

I have few more questions if you don't mind.

1.I did some research on the internet , and there was a mention about possibility of smegma if a swelling with white stuff is seen.. what makes this photos look different from smegma?

2.what signs should i look for incase of infection? many websites states that yellow discharge and swelling could be sign of infection , however these two things are already there in every circum... so what different or characteristics should i see to consider possibility of infection?

3.it's been 7 days since the circumsicion day, the ring is cut apart with little part that is free from the ring and the rest being encircled with it, how many more days should i wait for the ring to fall that if it doesn't fall after these days i should take him to doctor?

Maintain good penile hygiene.

Hi again.
There's nothing to worry.
You're bathing him well. Avoid any moisture remaining there.
Then apply the ointment.
Smegma will not form in children, if you maintain good penile hygiene.
The rest of the ring should fall in the next few days.
