What Causes Painful Urination?

Question: 54 year old male. I had brachytherapy one year ago for prostate cancer. Things seemed to be going along fine - taking flomax 4 mg per day. For the past three months, urination has been very painful, and urine is typically a very dark yellow solution that sinks to the bottom of the toilet with small particles that float. Sometimes a small speck of blood at the end.

In the summer I began exercising frequently, including long distance running. I mention this, because I know that long distance running can lead to appearance of blood in urine and it is innocuous. Also dehydration.

Tests for UTI and standard STDs were negative.

Please let me know whether you have been having any follow up tests on the prostate cancer like Free PSA to Total PSA ratio or an ultrasound examination.
These would also help confirm that the blood in the urine is only due to the long distance running and has nothing to do with the prostate.
If you have these results, I would be interested in seeing them

PSA before procedure was 3.26. Last May it was 1.4. I think last test (in October) was 1.1. I have not had any other diagnostic tests since a month after the procedure (CT Scan and MRI).

I suggested cystitis, but Radiation Oncologist doctor thinks this is unlikely given lack of fever and other symptoms. Told me to wait and see. I suppose dehydration is a real possibility and I should drink more water.

Glad to know that the PSA is coming down. A simple urine microscopy which can be done in any lab will help resolve whether you have an infection or not. If there is no infection drinking more water would suffice.
