What Causes Painful Urination?

Question: I have had NSU/NGU since mid July after an encounter with a lady who had a productive/phlegmmy cough. In hindsight that should have been enough of a reason to not proceed with anything.

We did not have any intercourse, she only performed oral sex on me as I have on her, however when she did perform it on me she had coughed into my penis. The second thing was her rubbing her private parts against mine later on (no intercourse) and that caused a lot of friction and irritated my urethra.

(The only possible other explanation would have been the use of one of my sex toys some weeks back which I had not cleaned well prior to use, however I had never shared it and I had no sexual infections before that ever.)

Two hours later I felt a twinge/sting in my urethra. I had decided to go to a GUM clinic two days later and get tested. Results came back clear.

I waited another two and a half weeks and did the same, results came back clear again. They tested for Chlamydia and Gonorrhea.

I was told UTI soon after by my GP (and it wasn't UTI as I've come to realise) and was given a course of Trimethoprim for 7 days. That did nothing. Afterwards he gave me 14 days worth of Ciproflaxacin. Also did nothing, the pain in my urethra had travelled to my testicle by then.

I had an ultrasound done, that came back fine.

I then decided to finally see a Urologist and he diagnosed me with NGU/NSU and gave me a course of Minocycline for 2 weeks. By day 10 it had gone away. I also had a Cystoscopy to ease fears of any stricture, there was no stricture. I had decided to also include Doxycycline in that regimen for the last 6 days of that course.

By day 16 (two days after finishing the course and the day after my anti-reflux hiatus hernia surgery) the problem had returned so I was given a six day course of Zithromax and Naproxen, I had finished three days ago.

The problem had eased a bit but I still have the tingling sensation in urethra to some extent and a little bit of a burn sometimes.

There was never any pain when urinating since this problem has started.

I am thinking of taking Doxycline for 7 days again. Do you think this is just urethral inflammation because I took too long to clear up the germ? Or do you think this is a reinfection? Or do you think I have been on the wrong antibiotics all along?

I have been tested for Chlamydia and Gonorrhea twice and both came back negative. I have no discharge, ever.

I have not had sex since the encounter with that lady. And we never really did intercourse to begin with so I don't think this could be something like Trichonomas.. I think its Mycoplasma but surely that much antibiotics would have cleared it by now?

- It is urethreal infection/ inflammation.

- It is adviced to take a antibiotics like azithromycin and doxycycline in combination for the same duration(Vibramycin-D) taken previously.
- The drug prescribed to you previously was right but some times there is chances of reinfection.

- NGU has some other causes also apart from the chlamydia. Gonorrhoea is not included in the list of NGU.

Hello Doctor,

Yes I am aware of that, however if it is not a reinfection and it's just inflammation because I took too long to treat with the right antibiotic then how long would it take to subside?

I am already taking Naproxen and Doxycycline again starting today.

Drug resistant NGU, extended therapy needed

Hello,

- There is no possibility of inflammation alone as you are taking a Naproxen, (Non steroidal anti inflammatory drug). So inflammation has definitely subsided in your previously taken therapy.

- You should think of Vibramycin-D (Azithromycin + Doxycycline) and metronidazole with Naproxen, it is a extended therapy for drug resistant NGU. Please take the above drug under your doctors guidance.

I am already taking Doxycycline again (I have already finished 6 days Zithromax before that), however not sure about Metronidazole as they have screened me and no Trichonoma was found. I can get a hold of it anyway, how long do you think I should be on metronidazole?

- There is possibility of recurrence of NGUs in you. If the previous NGU is recurred it is presumed that the organisms is resistant to the drug prescribed previously.

- Combination therapy will help to eradicate NGU.

- The treatment you have to take with due consultation of the doctor. Usually 14 days of treatment is required, but you can decrease or increase as per your doctors suggestion.
