What Causes Painful Swelling Around The Anal After After Injecting Triamcinolone In The Scar Tissue At The Surgical Area After A Fustulectomy?

Question: Hello Doctor,
As you re already aware that i had a fustulectomy in May 2016 and after the scar formation in August there was a consistent pain near the surgical area.
On this Saturday I visited a plastic surgeon and after examining he confirmed that the I have a thick scar tissue and he injected Triamcinolone 40 in the scar tissue, since then my pain in scar area has gone but the entire area inside my anus pains like hell and is swollen.
When I consulted the plastic surgeon he said that it is strange and wait for some days.
My condition has worsened . Please suggest is it normal to have swelling after this injection and will it subside or the damage is irreversible .

Noted the history well and understood the concerns.

Since Triamcinolone is given in the scar tissue pain is like to occur but swelling and persistent pain is not known to occur.
It is not normal to have swelling after injection.
Usually this should subside.
I would advise you the following in such a situation:
Consult a General Surgeon to get an actual examination locally, per-rectal digital examination and ano-proctoscopy only if the pain is bearable otherwise to wait till correct time.
MRI of the area can help o know the exact thing that is going on inside if actual examination is not possible due to pain.
A course of an antibiotic and anti-inflammatory medications with painkiller property may help.
Take sitz bath for warm fomentation.
Take stool softener.

Post photos if the swelling is external.

Will get the check up done . Can u please confirm if the injection has reacted then what is the time frame for the effects of injection to get settled. Or the effect will be permanent.

This being a steroid itself is not much to cause reactions.
This acts for about 4 weeks.
The the settlement of symptoms will depend upon the the effect it has cause, whether local reaction or infection and so on.

Hello Doctor,
went to a surgeon to get the check up done as pus started coming out from where i was having pain after the injection.
find attched the MRI report. doctor has suggested surgery.
please share your thoughts.

Since the pus has come out and the MRI shows short inactive tract as mentioned a course of an antibiotic should help to resolve the issue. This has occurred after injection - self explanatory.
What type of surgery has the Doctor suggested.
Has the pus stopped coming out? How is the pain and swelling now?
If possible post a clear and focused photo.
Also take an anti-inflammatory and other things as discussed above.

Pain is very very intense,cant even touch the area.
Pus is still coming out.
I can also feel some kind of swollen pockets inside the anus which is very painful to touch.
Doctor just told me that we need to take out the pus i dont know what surgery he is suggesting. He has mentioned fistulectomy on admission form for estimate.
Pain is unbearable

If the pain is unbearable, feeling of pocket, pus still coming out then incision and drainage can really help a lot.
Most of the decisions in such a case depends on on-table findings.
Please go ahead for surgery after discussion with your operating Surgeon.
Also discuss with him about the possibility of excision of the fibrosis/scar.
