What Causes Pain While Walking Inspite Of Having Mild Lumbar Stenosis?

Question: Dear Doc - My mother is 70 years old , diabetic (insulin dependent) and CABG . She has mild lumbar stenosis . Since a month she gets extreme pain while walking - more so from the fact that she leads a sedentary lifestyle . She undergoes physiotheraphy at home on a daily basis . Sugar is well under control . She takes ULTRACET for her body / joint pain . My query was is there a better way to manage her pain ? Her consulting ortho was recommending a supplement jab on her knees , which I am not in favor of (considering her complications)

I read about these medicines and was wondering if it is safe for her considering she is also a CKD patient .
Painkiller :
THRIZE - Trypsin BP 48 mg; Bromelain 90 mg; Rutoside Trihydrate BP 100 mg

TOLAGIN (4/8) - A combination of Aceclofenac, thiocolchicoside, and paracetamol presented in 4 and 8 mg strengths in strips of 10.

Supplement
Tendofit -(Collagen Peptide Type I, Sodium Hyaluronate , Chondroitin Sulfate & Vitamin C Tablets)

Can you please help me understand and advice ?

Thanks for posting your query.
Your mother is having a disc prolapse with stenosis of the foramina in lumbar spine from which nerves come out causing the nerve compression.
Right now she has got only one symptom- pain during walking.
The treatment is done according to the severity of symptoms. Medicines are the first line treatment along with physiotherapy. If the patient responds well with medicines then physiotherapy helps in preventing further progression of the problem.
The main treatment of such problem is bed rest along with pain killers and muscle relaxants. Muscle relaxants can help with her symptoms if used in appropriate dosage in combination with a potent analgesic. She can start with Tolagin 4mg twice a day after meals. This can replace Ultracet that she is taking now. Thrize is used to decrease the swelling around the joints and the foramina. This will also help with her symptoms in a twice daily dosage. Tendofit is a joint lubricant supplement which will retard the osteoarthritic changes in her joints. She needs at least thrice daily supplements of these lubricants. You can get these prescribed from her orthopedician.

Local analgesic gels and hot fomentation may provide a short term relief. She can use analgesic gels like Volini gel. Exercise is not advisable in the times of acute pain.
Using an LS contour belt of appropriate size might be helpful in providing support to back muscles.
Physiotherapy also plays a major role in treating such conditions. Epidural steroid injections have been used if oral pain killers are not very effective.
Discuss with her orthopedician and an appropriate treatment can be planned depending on the response to medical drugs.
