What Causes Pain Near The Anus During Defecation?

Question: Sir/Ma'am,

Most often, I feel severe pain on left side of anus while passing stool. It gets rectified on its own but it again comes back after few days or may be weeks and affects only the left portion of anus with mild swelling. I also feel sometimes small piece of skin/tissue hanging at the back of the anus. The pain is always external/outside and not on the inner lining of anal cavity.
I would be thankful if you can kindly let me know the name of the disease with which I am suffering and proper remedial measures/treatment. Also, I would like to know that why it always affects only the left portion of anal cavity.

It seems like you are having an Anal Fissure.
If you have constipation then it can lead to a small cut in the anal opening and that is called a Fissure.
It heals but if constipation or hard stools happen then it can happen again and again. To protect from the cut area being exposed to pain the body grows a small piece of tissue called a Sentinel tag. That would be the small piece of tissue you feel.
You need to get examined by a Surgeon to confirm the diagnosis and then the treatment can be started.
Treatment consists of medicines to soften your stools for at least six weeks like Isabgol or Senna or Tab Dulcolax. Then pain killers like Combiflam and antibiotics if there is suspicion of infection. Also, Lignocaine ointment to ease the pain in that area, before and after passing stools, as many times you want to apply. Sitz baths - Sitting in warm water collected in a broad container also helps to ease the pain. This condition also is caused by increased pressure in that area which can be relieved by GTN ointment applied twice a day. However, the best solution to this problem is Surgery in the form of Lateral Sphincterotomy. But, for now you can try medical treatment and see if it relieves you.

Sir,

GTN ointment which you referred in your previous mail is not available in the market, instead a ointment named rectogesic is available. is it the same?

Also, will the sentinel tag too go away with this treatment or it requires surgical treatment?

Rectogesic has similar composition and does contain GTN. You can use it.
As for the sentinel tag, it will not go away with this treatment. It will stay. If you want it removed then it can be removed only surgically. However the tag will not give you any problems even if not removed.

Sir,

Rectogesic ointment is also not available in the market. I request you to recommend some other ointment for this purpose or the place from where I can get this particular ointment.

There are lots of companies making this product in XXXXXXX It can come by the name of Nitrogesic or Myovin also. You could also ask for Diltiazem ointments if GTN ointment is not available. You could find these online as well for ordering.
