What Causes Pain In The Right Side Of The Chest?

Question: I am 44, male professional. I had pain in the right side of the chest, just below the collar bone 4 days ago. The pain, very mild spreaded to the whole upper part of the chest. The doctor has prescribed ofloxacin(400mg daily dose)-ornidazole combination. After taking 5 doses, due to hyperacidity I discontinued the medicine. Now I am feeling some mild pain in the whole chest, feeling feeble pain while swallowing. There is no inflammation in throat, no fever, no cough, no running nose . There is cold, winter in my locality. I had also a cold exposure a day before.
Eosinophil- 7%, no urine infection, random sugar 70 %, BP 136/90
Rest are in normal range
please advise...

Hi,
This is Dr Sameer, cardiologist from Fortis hospital.
Pain in throat while swallowing & associated chest pain could be because of Upper Respiratory tract infection for which you were prescribed oflox & ornidazone. If it is causing hyperacidity then I would change it to Azithromycin 500mg once a day for 5 days. Continue pantoprazole & if acidity is very much, you can increase it to twice daily(half hour before breakfast & dinner).
Also get a chest Xray & ECG done to rule out other causes of chest pain.

Hi,
Just went through your reports. Vit D3 is low for which you are already taking supplement. But one important think is you have albumin in your urine which denote early kidney involvement by hypertension. So take care of your bp, take regular medications.

The Vitamin D is 9.4 ng/ml.
Eosinophil is 7 %
Albumin in urine is 15 mg/dl

My consulting physician has prescribed -
1 ) Cholecalciferol 60000 IU - twice weekly for 4 weeks, then once a week for 8 weeks.
2 ) Vitamin D3- 500 IU + Calcium 500 mg tab- once daily for 60 days.

Are the above right doses of Cholecalciferol ?

The physician apprehended that the elevation of Eosinophil might be due to worm infestation. He has prescribed albendazole(single dose) for 2 consecutive days.
For the BP he has advised low intake of salt during day, no salt in dinner. After some random measurement of BP after some interval he may prescribe any medicine, if required. Should I go for RE/ME of Urine at a different lab for confirmation ?
Are the medications OK ?

Hi,
The dose for Vit D3 is perfect. Continue it accordingly.
As your bp is not that high your doctor has not prescribed you any medications and rightly decreased your salt intake.
You don't have to do whole of urine R/M. Its better to do "urine for microalbumin" only.
About Albendazole- In XXXXXXX we recommend it to take every 6 month because worm infestation is very common here. So you can safely take it.
