What Causes Pain In The Groin Area With Nausea And Back Pain?

Question: lower back pain, to groin area and testicles, sometimes upper flank and some nausea,comes and goes but has been persistent lately. right side , ultrasound and x rays normal

Since how long you have this lower back pain?Is it on the right/ or left side?
Do you have any diarrhoea/ Constipation Complaints since the inception of this pain / or lately?
Is the upper flank pain accompanied with fatty/fried/ non-veg food,after partying/ or after street food feasts?
Did you get any treatment for this pain on right side?Update the list of investigations done-especially-Urinalysis/ and Stool-Ova-Cysts with blood CBC and eosinophil counts?
Is the pain with colicks and spasms on right upper flanks?
Do you have accompanied chest burning / or back pains?Do you have bloated abdomen along with eat, specially after eating diet?
Since when it has become persistent-How long and was it after feast of non-veg/ fried/ fatty food?
Are you overweight/ or obese?
What is your frequency of tea/ coffee/ or beverages in a day?
Update details of your -typical diet in a day?What is your fluid intake-in a day?
Do you like dairy and or / bakery products?what is its frequency in home made / or ready food you eat in a week?

IN the given situation, You seems to be having-persistent problems of pain with nausea in right upper flank.
In the given situation,I would suggest you to give more details as asked you so as to rule out Other causes with more investigations,before start of any treatment suggested herewith

Follow up: Dr. Mukund Savaskar (10 hours later)
no issue with regularity, no constipation, no diarrhea
lower back to testicle and flank pain on and off on right side over 1 month.
Dr. gave me Tramadol, only took one and it got me nauseous,
Advil doses 3 times a day seem to help the best at this point
coffee 2 cups per day --7-8 glasses of water, 2-3 beers per day.
Bloating after eating does seem to be a problem lately
very little bakery products , only eat whole grain bread maybe a little too much butter at times

Read and reviewed your query updates in context to your health concerns.

My opinion-
In the given situation-You seem to suffer from-IBS with Chronic dehydration.
Comments and reasoning of your health issues-
The right flank pain with back pain is mostly from IBS right colon with bloated feel lately.
This is mostly caused from the food and fluid intake which is comparatively less to compensate for 3 beer intake.
Lot of bread in diet/with brewers yeast is causing bloating lately.
The pain the back with right flank pain could be from right colitis.
With much butter in diet,chances of Gall bladder bile sludge -which is nonvisible on USG is mostly causing back pains with right flank discomfort.
Accompanied GERD from all this food and lifestyle stress, could be adding back pains.
For the pain being referred to right groin / and testes,needs to be investigated by Consults with General Surgeon/ Urologist if need be.
This could be from accompanied-Varicocel/ or could be from PID(prolapsed intervertebral disc) at L1 vertebra-level with nerve compression causing referred pain to L1 -deramtome ie -supplying right groin and right testes.
Possibility of accompanied STD causing testes pains needs to be ruled out, before starting any treatment in your complex story, which needs to be investigated with above angles.
Other causes need to be ruled out after Consults with Specialist General Surgeon/GI Surgeon /Urologist and or orthopedic Surgeon-doctor -as need be.

Do's Suggested-
Investigations-
Get CT Abdomen and Spine if need be to study Gall Bladder Sludge-from much butter consumed/ and to rule out-PID causing groin and testicle pains.
USG with doppler study to rule out-Varicocele-is advisable.
Get stool test for Ova/Cyst and Urinalysis-after consults with General Surgeon.
Also get proctoscopy to rule out any piles /or anal fissure,causing IBS bloating.

Diet and Medicines-
Avoid beer intake.
Be on bland rice porridge initially and thereafter /or vegan diet x 3-6 weeks.
Plenty fluids to recoup for 3 beers you took.
Youghurt x 2 times a day.
Butter Milk x 2 times a day

All medicines with script from family doctor-on following lines -as needed
Tab Lactobacillus-1 x 2 times a day
Tab Dicyclomine-1 x 2 times a day.
Tab Multivitamin with Mineral- 1 per day.
Tab Vit B12/Folic Acid to recoup chronic needed.
Tab Ibuprofen(Advil) -to be continued after orthopaedic consults, if needed.
Avoid Tramadol-as its causing nausea and is not accepted by you.
Accompanied Constipation-if any with all this, needs to be treated if need be with medicines.The above dietary and lifestyle changes would stabilise bowel movements in your case.
