What Causes Pain In The Center Of The Chest, Dizziness And Excessive Sweating?

Question: Hi , I am a 33 year old male . Weight 91 kgs height 5.8 .
Firstly I was overweight so I. Started dieting since September 4. 2016 ... on October 12 I got viral fever. That time my weight was down to 85 kgs. I got blood test for chicken gunya and dengue and both were negative . As my fever went away in a few days. I got cold and cough. Till now things weren't worrying .. but on 25 th October morning as I woke up from bed and stood up.. I felt pain in the centre of my chest with dizziness and sweating.. I lied down immediately...this pain did not go away .. when I bent down or Turn a side on bed it would pain .. finally on 29th October I went to the doctor who got my ecg done.. in the ecg my pulse came 120.. so the doctor gave me indral and pexep 12.5 cr for axiety... same day night I got sweating so I went to Er .. there my ecg was normal and they did troponin t test which came negative .. so I was told I have acidity.. was given mayospas and pantocid for 5 days ... I felt relief from the pain the next morning .. but on the 4th day of taking these medications a new thing started .. I started having chest sensations .. and when I press the area of sensation I burp.. these sensations are mild sometimes and sometimes strong .. happen all over my chest and back.. sometimes on my neck ...I visited my doctor again he gave me meganeuron and rifagut for 10 days.. no relief he told me to get echo stress Tmt test .. which came negative . Now I was given less pitied and ganaton total for 10 days... no relief I went to a gastroentologist who did endoscopy and said I am Hp positive .. I have been given a medicine course for 2.5 months which had pantoprazole, amoxicillin and clarithomycin for 15 days ... and esomopiride magnesium and levisulpiride for 2 months ... almost 1.5 month past I feel better than before .. but I still don't feel 100% better ... I was told to make lifestyle changes .. so I started walking for 1 hour everyday since past 50 days ... my weight is down to 80 kgs now .. I have not eaten non veg since past 50 days .. I am avoiding outside food completely.. but whenever I eat I feel strong sensations and burping ...my question " the pain I got in the centre chest was related to my heart ?
If it was not related to my heart could it be costocondritis?
I have strong pain on my left side ribs .. and same result I press and I burp ..
Is my gastro treatment agreeable .. should I carry on with it .. or consult someone else ???

Firstly thanks for choosing health care magic for your query!
Have gone through all your details.
1)As per your question no most likely your pain is non cardiac in origin,your ecg's and Trop's were normal,Your echo and tmt test were normal.Combined together the sensitivity of tmt and 2d echo to rule out coronary artery disease is more then 85%.However for complete cardiac clearance a non invasive test called stress thallium should be done(though not indicated in your case). Stress thallium is 98% sensitive to rule out CAD.

2)Yes it could be costochondritic pain also.
Inflammation of the cartilage that connects a rib to the breastbone (sternum) causes costochondritis,and the pain is usually present at strenocostal angle(Rib and sternum junction).

The pain associated with costochondritis usually have following feature-
It usually occurs on the left side of your sternum,Pain is usually sharp, aching or pressure in nature,it affects usually more than one rib,and worsens when you take a deep breath or cough.However physical examination is necessary to make costochondritis as a diagnosis,usually your physician will look for rebound tenderness along costosternal angle which is almost always present in it.

You have a strong history of gastritis and acid peptic disorders ,endoscopy also revealed infection by H.pylori,The regimen for H.pylori is given for 3 months and should be followed as per the advice of gastroentrologist,Never stop medicines by your own other wise resistance may develop in bacteria.As you are relieved by the treatment you should continue it for atleast 3 months,Gastritis,GERD,and other acid peptic disorders mimics cardiac pain.Eating lots of fibres and vegetable also cause burping and flatulence,Restrict amount of fibres to 30gram/day.

Final note-
After going through your complete history,Most likely cause in your case is-
1)Acid peptic disorder and GERD-( For which you are on medication)
2)Costochondritis-For which physical examintion is necessary.

Advice
You can take tablet ultracet twice daily for 3 days ,if pain gets a bit better it can be costochondritis(consult your cardiologist ).

Hi,
Thanks for your reply.
On your advise i took ultracet tablet twice yesterday.
Point to be noted :- a few hours after taking it the sensations vanished and no burping...
In the evening as the drug effect diminished i felt a few sensations right before i took my second dose and after taking again the sensations and burping vanished ..
So should i continue with ultracet for a few more days ??

Yes continue it for 5 days and follow up after that!

Hello again,

Yes continue ultracet twice to thrice daily for 5 days (consult your cardiologist) , Its a very safe drug. If you are responding to this drug means that most likely its costochondritis. Ultracet is a week drug and is a drug of choice in your case as it will not increase your gastritis. However a controlled therapy with NSAID's and low dose steroids can be considered once diagnosis is confirmed.
