What Causes Pain In Right Epididymis After Ejaculation And Work Out?

Question: Hi,

Symptoms :
1. Mild/sometimes more Pain in Right Epididymis specially after ejaculation and work out.
2. Mild Pain at Particular area at corner of Right Testicle and start of Penis.
3. Feels completely normal sometimes during sometimes a day.
4. Warm heat around anus when seated straight posture on chair, it was more some days back and now it is less.

Questions:
1. What is the problem here?
2. Why is it not getting less?

Thanks for the query.

Chronic epididymitis is a condition which is little difficult to treat completely. Symptoms generally regress with supportive treatment but would reappear. Frequent follow up with Urolgist is necessary not only for confirmation of diagnosis (by completely ruling out other causes but also for proper planning of treatment. Antibiotics are needed frequently based on investigations results.

Hi,

Why did it occur?

Never had sex. Is it because of UTI.
What exactly happening inside the groin area when I am facing these symptoms.
Why is it difficult to eradicate ?

Inflammation of epidydimitis causes the symptoms you are experiencing. Treatment is difficult due to chronic nature of the disease

I would like to answer each of your question directly.

Being its nature of chronic, everything related to ejaculation , sex should be fine?

Everything related to ejaculation should be fine, does not affect this part. However,
anxiety due to pain may rarely interfere with sexual performance. Apart from that present condition does not affect sexual performance.

What is causing this inflammation which recurring and sometimes sharp ?

As discussed previously, infection(from bacteria) generally causes this.

I do karate that should be fine ?

Activities that aggravate pain should always be avoided. You on your own should identify such activities.

Inflammation will eventually fade out in time but cannot predict exact time. Please be reassured.

Hi Doctor,
Thanks for the reply.

1. Why is it chronic in nature?
2. Twitching, Mild to sharp pain, Warmness, or feeling of mild/sharp movement or something lying from upper side of testicle to the crossing of penis and pelvis( I am not how to describe the symptom)
3. Warm sensation during ejaculating on penis and around anus.
4. Feels normal sometimes and suddenly symptoms arises and then fade.

All these symptoms are expected in Chronic Epdididymtis.

This is the first time happening to me and it's kinda taking my attention largely.
Also for the matter of fact it's related to Reproductive system , I am bit worried.

According to Urologist there is nothing wrong inside. Ultra sound of Testicles, Pelvic and kidney does not show anything.Urine test with no infection.

There is no specific reason for it being chronic. And it is a diagnosis based on the symptoms of patient rather than investigations results. No wonder that investigations results are normal. It is common for this condition.

And all the symptoms you have described are known to be associated with this condition.

Ejaculations would sometimes aggravate symptoms.

I can understand your concern and I am really sorry about your problem.

As I have mentioned previously, this is the difficulty with this disease. Even when you are getting antibiotics, problem persists and symptoms may also continue. But treatment should not be stopped. And as I have also told, symptoms would eventually fade away but cannot predict exact time, varies widely.

Hi Doctor,

Why is warmness in anus during ejaculation and itchiness at lower testicle or feeling something mildly scratching back and side of testicles during ejaculation?
This symptom is less on normal time but during ejaculation it increases.

Also slight burning sensation on left side once in a while. Does is it mean it is spreading to left?

Also I noticed ejaculate is more clear liquid with less white.

No, it doesn't mean that it is spreading. The symptoms of this condition are very non specific. All these symptoms could be part of chronic epididymitis and it is not a malignancy to spread. Semen being clearer may not be related to this condition. However, as you noticed it, a semen analysis can be helpful to analyze further.
