Question: Hi i have perineum pain its also tender to touch is can this be prostate cancer psa 0.55 free psa 0.22 but i know psa in not accurate . I dont drink or smoke

Thanks for your query ,based on the facts that you have posted it appears that you have infection of Prostate (Prostatitis)

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Have you consulted qualified Urologist very recently and got Digital Rectal Examination (DRE)done ?
2) Do you have urinary symptoms like frequency,Urgency ,burning urination etc ?
3) Was Ultrasound Scanning of abdomen and pelvis done very recently?

Serum PSA is a important tool to suspect cancer of the prostate and is the base for need to do further tests like biopsy of the prostate to rule out or confirm the cancer of the prostate .With present value of your Serum PSA (0.55) it is most unlikely that you have Cancer of the prostate.

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Repeat Serum PSA.
4) Swab test and culture of a urethral discharge obtained after Digital Rectal Examination.
5) Endoscopic Examination (Cystoscopy) if required .

In the mean while start taking antibiotics like Doxicycline along with anti inflammatory medication like Diclofenac twice daily .

Further treatment will depend upon result of these tests and final diagnosis.
