What Causes Pain In Lower Abdominal Region Along With Constipation?

Question: I had mild pain in my lower abdominal region, went to a doctor days later after it had subsided. Said it didn't seem like diverticulitis, no cause found. Past couple of days, started experiencing similar symptoms again, but now, it's all focused on my lower left abdominal area. Pain, more than moderate at times, hurts when I move, tender to the touch, possibly some constipation. Had been worse during the night and debated on going to ER. No fever, no vomiting, no blood in stool, etc. though. Recent diet was poor, but now eating yogurt, getting fluids, etc. Debating on need to go to ER or urgent care, or follow up with doctor on Monday? Thank you.

P.S. I can at times feel like 'digestion' (fluid movement) is going on in the area as I just did (which made me think to add this).

Had thought maybe muscle sprain, but now wondering if diverticulitis, or GI infection, or ?
One other thing, I had been sitting, and now just going to stand, it's difficult for me to stand fully. When I go to stand up straight, I feel pain in the lower left area mentioned.

I have gone through your query and understand your concern.
From your detailed history, it looks like a case of stomach infection and indigestion. A muscle sprain might be there but should come after stomach infection if their probability is compared.
If such a patient comes to me, I generally advise the following-

Modification of diet is must. Avoiding heavy, oily, junk food is required alongwith taking plenty of water, small frequent meals and more dietary fiber. Also, avoidance of alcohol and smoking is important.
Next comes brisk exercise in a regular basis. This enhances metabolism.
A course of antibiotics with ofloxacin-ornidazole for 5-7 days should be helpful.
Tablet pantoprazole-levosulpiride once daily in empty stomach for a month should normalise bowel habit.
For abdominal pain, drotavarine tablets will be helpful.
For muscle sprain, thiocolchicoside tablets will be advised if not relieved with above.

You can consult your treating or family doctor about the above and for the prescription of medicine.

Thank you Dr. Dattagupta, I will begin with your comments on food and exercise and implement those.

I did forget to mention that I have non alcoholic fatty liver disease, and also tinnitus. Don't know if the medications suggested would be changed due to either of these two conditions I have, should I see my GP tomorrow.

Again, thank you for offering me some sense of relief in regards to my current circumstances.

Follow up: Dr. Swarnava Dattagupta (24 hours later)
Follow up: went to ER last night. Cat scan done, diverticulitis, mild, small. Was given scripts for Cipro and Flagyl. Concerned about Cipro/fluoroquinolone drugs because I have tinnitus already. Had taken Doxy years ago and developed tinnitus at that time, not to mention horrible side effects for me. Is there another effective antibiotic I could go back and inquire about? Eg, I've taken Amoxicillin in the past with no noticeable issues. Regarding sulfa based drugs, they are listed as an allergy because numerous years ago I developed an itchy, red rash on my chest (but no other issues). I've read very concerning stories of fluoroquinolone drugs and tinnitus, and there is no way I'd handle aggravating my existing tinnitus. Thanks, XXXXXXX

Hello.

Yes your concern is quite apt as fluoroquinolone group of drugs including ciprofloxacin might be associated with tinnitus. So its better for you to avoid it.
Plus you have also a history of allergy to sulfa drugs, hence trimethoprim-sulfamethoxazole combination is better to be avoided too.
Both are excellent drugs for diverticulitis.

You are left with a 3rd option of amoxicillin-clavulanic acid. It is a very good drug too and works well in diverticulitis. Alongwith, bland semisolid or liquid diet for a few days is advised.
