What Causes Pain During Defecation And Large Stool?

Question: Hi I am wanting to know weather or not I need to go to see a doctor for my condition if its something I should really be worried about yet or not. I have done some research into what I think is an "Anal Fissure" which I can only think I got from a large stool. It has become quite painful now and dosen't seem to be as easy to go away as when I had it a few times before, was around for maybe a few days I believe. It has been around for maybe 2 weeks give or take and there is a lot more blood then previous maybe and inch or two wide on tissue afterwards. As this is such a sensitive are and thing to occur as a 28 year old male honestly the last thing I would want to have to do it go for something like this as I believe also it could be a sexual preference kind of thing also (Im straight). I have been trying to have more fibre in my diet also and the stools seem to be quite darker then normal. I have also been taking Dexamphetamaine since about 4 months which I can think is the only thing that mite have contributed.

I tried to get a photo like what I saw on google but it was not exactly the easiest of things to do by yourself.

I did used to experiment with a prostate massager that was manual but not often, not long and not for a while either, each time with lube and dont think I really notice this come about because of it. I guess I should add. Im also not too sure what the white dots are in the photo I washed just before also lightly. I also seemed to have for the first time which was the worst i have ever by quite far very dry glans on my penis that was even peeling quite a bit and some redness, but I got that under control with a change of soap but that was around for a few weeks and is 90% gone now I think which I'm not sure that would be related or not. Thanks please let me know if you have enough to go on, it also didn't feel as bad then as I thought it would.

Yes, you should consult a General Surgeon for proper re-rectal examination and proctoscopy (when the pain is reduced to allow examination without pain), examination of glans, investigations and appropriate treatment.

I would advise in addition the following:
Sitz bath in a tub with warm water
Mild laxatives - adjust the dosage to keep the stool soft and do not worry about weight, will not be affected.
Continue with high fiber diet.
Observe/remember if any of the foods or beverages is causing hard stools, if so then stop consuming such things.
Clean the area with plain water, no soaps or any other chemical containing things other than solution povidone iodine (different swabs for peri-anal area and glans)
Stop using prostate massager
Avoid stress and/or anxiety.
Learn to relax the anal sphincter at passing stool; use Sitz bath just before want to pass stool and after passing stool and use water to clean.

If you maintain these things , you may not get the fissure again unless the previous instigating factors prevail.

Now about other observations:
- The condition of the glans you have is called balanitis.
- Clean with plain water, apply solution povidone iodine.
Anyway it is recovering as per your history.

The balanitis, recurrent fissure, the picture showing the excoriation of the intergluteal skin and the thinning of the peri-anal skin indicate the need for vitamins supplements. Take multivitamins and Therapeutic dosages of Vitamin A and D (after the blood test)
The regained integrity of the skin will increase the resistance power, better and may be permanent healing.

The white flakes are not significant, can be the clothe fibers or tissue or so as they look different from the skin.
Dexamphetamine can cause constipation.
