Brief Answer:
Can be nipple crack.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.

Sore nipple in lactating mothers may be due to injury to the breast tissue like by pinching or biting by the baby while feeding, infection of the breast - called mastitis, but is less common and is associated with swelling and redness. Pain in the underlying muscles or ribs may be mistaken for sore nipples and also abrasion and also rough clothes that she might be wearing.

She can try to eliminate the soreness by wearing light cotton clothes that will not rub against her nipples. Apply a lubricant like baby oil or any moisturizer over the nipples.

The crack will gradually heal in next 3-4 days. In case there is acute pain or swelling then do consult a gynecologist for a physical examination and stronger medicines.

Please feel free to discuss further. I will be glad to answer the follow-up queries that you have.
Wishing you good health.

Regards.
