Question: Hi! I am a 30 yrs old female with a 6yrs old daughter..I have been experiencing a mixtureof sysymptoms from past 6th. .my symptoms include oedema in the feet..sometimes all over the body..my BP is normal..Ian not a diabetic..i never had any pimples in my life, but now I got pimples..my skin has become very dry..hair fall is also one of the symptoms..With the dryness of skin Ian also experiencing genital dryness..my libido has reduced..my cycles are absolutely normal..but my PMS almost kills me. .I have become very irritable..my weight is totally normal..i have started getting unwanted facial hair growth..my USG shows that I am not ovulating..but my ovaries at completely normal..my hormone levels also normal as per my blood reports..I don't have PCOD but my doctor started glyciphage..but I had to stop it because of the side effects..Please help

Hello and welcome,

I understand your symptoms but they are a bit asycnhronised. You say that you have normal cycles but do not ovulate, have normal hormones but have increased facial hair. These are contradictory findings.

If ovulation is affected that means that there has to be a hormonal dysfunction. Most likely the imbalance is mild and is not detected by tests but you have a sensitive system that is not tolerating such a mild imbalance.

As of now most of the symptoms point towards one direction and that is problems with androgens. It seems that the symptoms are due to abnormalities in the androgenic hormones like testosterone.

One reason for these are ovarian tumors producing androgens but I assume that an ultrasound has already been done and ovaries are normal.

I am not sure which test were done and on what day of the cycle. As of now you need a serum testosterone and DHEAS as well as total androgens and sex hormone binding globulin evaluated on day 1 or 2 of periods preferably in the early morning.

I also suggest that you start tablet ginette. This is a birth control pill that has anti androgenic properties. So it will decrease male pattern hair growth and also control hair loss. Add vitamin b complex and folic acid to help prevent hair loss. Stress is a major cause of all the above symptoms so destress and also meditate daily. Other drug that needs to be added is spironolactone. This also decreases the male steriods and decreases hirsuitism. These need to be used for at least 3 months before effects are seen. If the hairs are stiff then other treatment like elctolysis or laser may be needed. These drugs will prevent further progression of the problem.

The drugs can be taken after prescription so you will need to see your doctor and discuss the options with him and get them prescribed. The PMS will also decrease with these.

In addition get an ultrasound of the abdomen done to look for any abnormalities in the kidneys as the swelling may also have a renal cause. A renal function test and hemoglobin as well as serum proteins need to be tested to look for the cause of the swelling. If there are abnormalities then they will need to be treated accordingly.

You will need to modify the lifestyle and add fresh fruits and vegetables to your daily diet as well as exercise daily for at least 20 minutes. If you over weight, weight loss will be very helpful.

So do not worry. Just begin the treatment and you will be fine.
