Brief Answer:
Could be cervical spondylitis.

Detailed Answer:
Dear Sir,

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that pain starting in neck radiating to shoulders most commonly due to cervical spondylitis which are basically age related changes in the spine causing compression of nerves .

Also diabetes needs to be ruled out as one remote possibility can be frozen shoulder starting . I would advsie you to get x ray cervical spine done and see a physician for the same . Till then for pain relief please ake paracetamol 500 mg if there is no contraindication for the same . You can use topical paracetamol gel over the neck also.

Take care.

Dr. Shruti
