Posted on Sat, 16 Apr 2016
Question: My husband had a massive heart attack on Oct 10,2015. During his heart attack he fell and hit his head on the hardwood floor. When he arrived in ER the doctor gave him TNK,he was then sent to CCU. After about one hour he had severe headache ,a CT scan was done showing he had four intercrandial bleeds. Rush him to ICU and they put him in induced coma. He has progress fairly well except right now he is in a rehabiltation center trying to walk. He is been limited because he has severe nausea, vomiting and dizziness. The doctors have prescribe him zofran, gravol, domperione and peridot, nothing seems to work. Can any doctor online help me, It would be greatly appreciated? We are getting very frustrated with this and my husband seems to be getting depressed.

Brief Answer:
Inner ear dysfunction, intracranial pressure raise also to consider.

Detailed Answer:
Hello and thanks for using HCM.

I have read your question and understand your concerns.

Rehabilitation after CVA is s very difficult and delicate matter.

Nausea, vomiting, and dizziness point towards an inner ear dysfunction, but can be also related to increased intracranial pressure.

I think there is need for a new brain imaging study such MRI, if there is no a recent brain imaging.

I think also your husband should be evaluated by an ENT about any possible inner eat dysfunction.

Funduscopic examination also needed to evaluate intracranial pressure.

In both cases ( inner ear dysfunction and probably increased intracranial pressure ) use of Mannitol 20%, for two-three days is indicated to improve your symptoms since all above drugs didn't control his symptoms and I'm assuming that he's having fair kidney function.

Hope you found the answer helpful.

Let me know if I can assist you further.

Best regards.

His nerurologist tells us that it's a long progress and his nausea and dizziness will resolve on its own. He said that the blood in his brain as to dissolve on its own and this could take months. The blood is irratrating his brain which in turn cause nausea and dizziness. He had a complete eye examination done eyes have no damage. Eye sight same as before his heart attack.ENT check for crystals in his ears but no other testing was done. He also had three laser surgeries on his voice box due to problem with breathing. He also as a trach in till the ENT sees in on his follow up nexts Friday. Blood pressure is low at times 94/64. Here is a list of medications he is on Atorastatin, metoprolol,Asa,Vitamin B12,Pariet,Zofran and Motilium. Do you think this is possible to have blood affecting the brain causing nausea and dizziness?

Brief Answer:
Blood pressure should be maintained within normal limits.

Detailed Answer:
Welcome back.

Blood pressure should be maintained at 120/80 mmHg levels in order for the brain to be supplied correctly.

Medications also could affect brain functions by side effects, but first blood pressure values should be corrected.

Presence of blood into brain is true that needs time to subside, but imaging follow up is needed because the possibility of brain edema in the areas surrounding hematoma.

Hope I helped you.
