Even though, with your history of migraine, it can justify being just a migraine episode that has not been treated completely. From what I understand you are getting relieved a bit from the use of Ibuprofen. This is a good new, because if it was a tumor or something more serious, you wouldn't be better by using it.

I would say add a tylenol between the Ibuprofen that you are using, for more relieve, and give it one more week time. If not better then consult a neurologist .

If nothing has changed, in terms of duration and intensity, and if Ibuprofen is helping you, then I expect for the pain to go away in a couple of days.
The location is not specific because migraine can change location.

-- If the area in the temples is sensitive and painful when you touch it, then you should go to doctor asap. You may need Steroids and Antibiotics if it is a Temporal Arteritis, and it is urgent.

-- If it was a clot in the brain, the pain would be much worse and it wouldn't get better better by Ibuprofen. You would also have other neurological symptoms that can be ruled out by a neurological examination.

-- About your brain lesions, I would say that you need to follow those up with another MRI, once/year or earlier depending on how they look on the MRI. This fact makes your headache more serious and you should be more careful if changes happen. No need to worry if you say that things are the same that they have been and there has been no changes.

-- So to summarize: Take Ibuprofen for couple of more days, don't let the pain aggravate before taking the next dose (so take them regularly every 6 hours), try to rest in a quiet with no much light place. It not better in 2 days then consider seeing a doctor so he can perform a neurological exam (its necessary and we cannot do it from here), maybe another MRI (that may be necessary to0 to understand what is happening with those lesions), and take treatment according to the results of those steps.
