Question: Hi, i have pcos since past 4 years. I amon metformin which usually helps. However, 4 months back i had a skin infection and so had to take Augmentin which has affected my periods andi have not had my period since then. I was adviced to take gynaecosid and i took it 2 weeks back but it has not helped. What do you suggest? Should i just wait?

Brief Answer:
get a transvaginal ultrasound done

Detailed Answer:
Hello,
Thanks for the query to hcm,
The fact that you have been taking treatment for PCOS and that you have been amenorrhoeic for last 4 months now with pregnancy having been ruled out. Then it is suggested that you should get a ultrasound pelvis done to check for endometrial thickness which if found more than 9mm, then you may opt for a withdrawal bleed with progesterones.
And if the menses still don't occur then you may have to get a dilatation and curettage done.

Dear Dr,

Isn't gynaecosid as good as Primolut-n? Is there any other medication you would prescribe? Or is it normal for pcos patients to have such a big gap?

Detailed Answer:
Hello,
Gynaecosid is an estradiol pill and not a progesterone pill which can initiate a withdrawal, hence not same as primolut-n.
For a PCOS patient, delayed menses is a known feature but absence of menses for 4 months is not likely. Amenorrhoea can be due to possible accompanied hypothyroidism as well which may be investigated for.
Hence an ultrasound is suggested to check for the endometrial thickness which shall give an idea of the hormonal phase in which the uterus is at present and if the ET is more than 9mm , a trial of progesterone withdrawal bleed can be given else a D& C would be the available option for you. Post which you can get a hormonal profile as well to look for hormonal deficiencies or excess.

At the moment i am slightly tied up with family and cannot carry out the ultra sound. Is there any progestrone pill you would possibly prescribe?

Detailed Answer:
Hello,
In that case, if pregnancy has been ruled out then you may use :-
TAB PRIMOLUT-N 5mg twice a day for next 5 days.
You can get it prescribed from your nearby physician.

Then you should wait for your withdrawal bleed within 7 days after the last dosage. The bleed usually occurs in next 7 days and likely to be heavy as well.
