What Causes Mildly Elevated Lymphhoctyes?

default
Posted on Wed, 5 Nov 2014
Question: For the last two years my lymphhoctyes have been mildly elevated; 11.400. Two months ago they went up to 13, 300
I feel healthy and have no symptoms, but a routine cat scan revealed a thryoid nodule, .9cm. I am having a needle biopsy today.
cat showed no lymphedema. I exercise regularly and am average weight.

I also take chlorella supplements. wondering if that has effect on lymphocytes

Brief Answer:
is it lymphocytes count or total WBC count

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Looking at the query, my opinion is as follows:

1. Increase particularly only in lymphocytes count is being seen. Need more detail on total leukocyte count and overall lymphocytes percentage.

2. Lymphocytes percentage of more than 80% suggest chronic conditions. Require other white blood cell percentage. If you are not sure, attach both reports.

3. Chlorella supplements could raise WBC count. Rather than lymphocytes, neutrophil might increase.

4. Thyroid lesion with inflammation could produce increased WBC count. Particular WBC increase depends on cause. If possible, get back with FNAC report.

Do attach report or give more details. If any peripheral blood smear report was done, it would be more informative.
Any further queries, happy to help again.

Take care

Background:
I am a 65 yr. old female about 5/7" 127 lbs. I feel quite well. Resting pulse is 57, blood pressure is about 115/ 75. I exercise vigorously,, no fatigue, no loss of appetite, very infrequent colds etc. BUT, for the last 2 1/2 years I have been experiencing ELEVATED LYMPHOCYTES

Medical Background last two years
1. Primary Raynaud's-diagnosed when I was 17 years old. ANA and Sgrogren's test negative
2. I had a uvulectomy 3/7/2012 (snoring not sleep apnea)
3. I had a kyphoplasty for compression fracture from weight bar 8/26/14
4. Benign thyroid biopsy 10/10/14 (hyperplasia benign)
5. CT scan 8/1/14 showed mild lung scarring probably from inflammation, no lymphendopathy

6. Since 2012 I have been undergoing major dental work, including periodontal, and two dental implants 7/30/14
7. April 26, 2013 I had stem cell Platelet Rich Plasma injected into my left TMJ joint to correct osteoarthritis (from severe grinding my jaw dropped 3 mm.)

LYMPHOCYTOSIS

1.1. 2011- totally fine 8,1000
2. 7/12 don't have paperwork. Dr. said just lymphs mildly elevated WBC 11.4
3..
. 7/30/13 WBC 9.9 LMPHS (absolute) 4.6 (in parenthesis dr. put (5.5 which is what I think they were in XXXXXXX 2012) all other blood work normal 0 and 0.0 immature granulocytes and immature grans
4. 4/21/14- don't have paperwork, but WBC about 11.2 only lymphocytes raised, and I developed Chalazion which was surgically removed. Put on Prednisone and Tobradex eye ointment for 10 days.
5.SEPT. 26, 2014 WBC WITH DIFFERENTIAL/PLATELET 13.3 x 10E3/ul
a. LYMPHS (Absolute) 6.6 x 10E3/ul
all else ok. 0 and 0.0 granulocytes and immature grans

My Primary Dr. was recommending watchful waiting. I think it is time to see a hematologist. I also have sacral herpes and I think I should take small amount of Valtrex to prevent outbreak? What do you think.

Brief Answer:
Repeat complete hemogram with peripheral smear

Detailed Answer:
Hi,
Thanks for asking again.

Looking at the reports attached, I can give my opinion as follows:

1. It appears to be lymphocytosis.

2. Increased lymphocytes should be chronic inflammation. At this age, we need to also think of chronic lymphocytic leukemia (CLL).

3. No immature cells are seen means possibility of CLL is low. However, 0 granulocytes is worrisome.

4. Lymphnodes should be enlarged at multiple locations for this amount of lymphocytosis.

5. Get a repeat complete hemogram along with a peripheral smear. If you find no granulocytes, you have to meet your hematologist. In the hemogram check for total WBC count, differential count and ESR.

6. You can take Valtrex for Herpes.

Repeat a complete hemogram with peripheral smear. I think this is completely necessary before your meet your hematologist. A report is absolutely necessary.
Do attach report.

Hope it helps.

Brief Answer:
Granulocytes, very important against infections

Detailed Answer:
Hi,
Thanks for asking again.

No granulocytes means no neutrophils which is not seen ever. You should be full of infections.
That's why, its worrisome.
No immature cells, no leukemia.

Repeat reports necessary. No lymph nodes, no malignancy or cancer.

Repeat hemogram test. At present, no leukemia. Attach reports, if possible.

Hope it helps.
Any further queries, happy to help again.

Dear Dr. Muddegowda. I gave you incorrect information before
My absolute Neutrophils are 5.6 x 10E/ul

It is my immature granulocytes that are 0%
and my immature grans (ABS) that are 0.0%

Brief Answer:
Double sure- no leukemia

Detailed Answer:
Hi,
Thanks for clarifying that information.

With this information, I am double sure, no blood cancer or leukemia.

Both lymphocytes and neutrophil are within normal percentage.
Meet and talk to your hematologist. No leukemia.

It maybe some inflammation which has caused rise in leukocytes like arthritis joints, etc.

Take care.
