Question: I am having trouble passing urine. I have to push hard for anything to come out. My testicals are extremely tender. I have low grade fever and back is killing me.

Pain or tenderness over testis with difficulty in passing urine might occur due to inflammation (orchitis or epididymo orchitis) or urinary tract infection or prostatitis which could be bacterial or viral infection but needs to be evaluated further by examination and some tests.

Your low grader fever with back pain or trouble urination are also indicative of some infective pathology which I would think as a first possibility.

Therefore,it will be important to get a clinical examination done of the testis (scrotum) with epididymis,penis and lower abdomen and then an ultrasound scan might be required to know if there is any problem inside the testis or to its nearby structures.

In conclusion,kindly consult surgeon or urologist who will take your complete medical history with thoroughly clinical examination which will include,

(i) Examination of testis,local lymph nodes and penis,
(ii) Rectal examination to rule out prostatitis or its enlargement,
