What Causes Mild Abdominal Pain After A Piles Surgery?

Question: dear Dr

i had done a conventional piles surgery on 1st XXXXXXX everything seems ok until 23rd XXXXXXX I m having mild lower right pelvic pain, some blood stain after farting on thursday and Friday.since thursday abit of blood stain on the stools. I went for review today, my Dr insist dear Dran colonscopy on monday for which i already done one endoscopy last year january and there is no problem with my report. Can help advise if a colonscopy is advisable after my piles surgery? Will it irritate my wound further? I m really confused now. i m 44 years old frmal from singapore

My Impression-

In the given scenario,You seem to suffer from-Post-haemorrhoidectomy procto-colitis with Constipation, leading to blood stains from farting.

Pain in right pelvic seems to be from hold up gases due to accompanied Constipation and Colitis.
Conventional piles surgery wound usually should heal by 2-3 weeks time.

It seems that-you have had street spicy food and contracted food induced IBS with farting of gases,from procto-colitis.

This procto-colitis has caused blood staining from hard stools stretching the anal piles operation wound, with gas farting.

Dear follow the following suggestions with which you would get relief.


Do' Suggested-in given situation-

In view of the food induced I would suggest to treat accompanying IBS with procto-colitis and Gases.
Be on Bland liquid,low volume veg diet and increase it gradually.
Avoid spicy,fried, non-veg diet..

Be on - Yogurt /Cap Sporalc-probiotics- x 2 times a day,to stabilize colonic commensal microbials.

Take antibiotics anti protozoals(like Ciplox with Ornidazole) x 1 weeks time
Tab NSAIDs to control proctitis
Anovate cream -per anal application for 5-7 days
Add PPI -Omprazole with Domperidone-to prevent accompanied acid reflux
Add Vit B12 and Folic acid
Gradually add low volumes of solids over next 10-15 days time.
If you get relief in 3-7 days time, just follow dietary restrictions for next 2 weeks , to avoid its recurrence and more blood stains.
If no relief after 5 days time, get anoscopy/ proctoscopy to assess the cause at operated site.

Key ALERTS-
treat accompanied Colitis as suggested above.

IF still no relief-

Consult Surgeon and get proctoscopy/ or anoscopy under local Xylocaine jelly,which would resolve the issue.
No need of colonoscopy,if you stick to dietary regime advised.
Plenty of fluids and care of the Constipation with stomach gases, caused from food induced- IBS with Colitis from accompanied -is the main stay,of avoiding colonoscopy in your case.

thanks for the detailed guidance given. my dr insisted a colonscopy for tomorrow. should i cancel or proceed with the colonscopy. will it be dangerous or any side effects if i proceed with the colonscopy?

Q-1- should i cancel or proceed with the colonoscopy.
Ans-Yes,for the reasons discussed below, You should cancel tomorrows Colonoscopy,as your health issues could be dealt with medicines and diet advised.
More than that, to assess the cause of blood stained stool,proctoscopy and anoscopy,would be sufficient and could be done if no relief after medicines and diet,after a period of 1 weeks treatment with medicines.

Q-2- will it be dangerous or any side effects if i proceed with the colonoscopy?
Ans-2-
Chances of reopening of the recent wounds from operated site and bleeding is the only risk.Besides this specific reason, the common risks for Colonoscopy could be avoided, by cancelling the untimely Colonoscopy.

In the given scenario,as things can be resolved with trial of medicines and dietary suggestions,I think you should cancel Colonoscopy for tomorrow.

You should postpone/ or cancel Colonoscopy,as the blood stains are from hard stools due to Constipation accompanied with procto-colitis which is caused from food induced infections,in your case.

Thank you doctor. i forget to mention that before the piles surgery, i went to do a rubber ligiation on 31may by another doctor which went wrong and causes gangrene as this doctor band on my skin. i was ordered for an immediate op by this new specialist. attach a photo showing the piles send to the lab for test and prove gangrerous and my blood test results shows infection. the few blood tests after the surgery showed infection was cleared. but this blood stain issue follow suit on 01june. could this be the reason why my new specialist insist of a colonscopy or this is a seperate issue? is it still advisable to cancel the colonscopy. very sorry for not informing you the reasons for the piles surgery

Read and reviewed the query in context to your health issues,related to cause of conventional piles surgery,after thrombosed and infected piles-due to unskilled rubber ligation / or banding by medical attendant.
Dear, even with this history, My reply would be same,as in such a situation,the wound healing would be delayed.This also suggests the right pelvic pain from post-op tight anal sphincter,causing pain from retained gases.
This also explains the procto-colitis, which gave blood stains after food induced entero-colitis in your case.

Reply to your 2nd specific question-
Q-1-could this be the reason why my new specialist insist of a colonscopy?
Ans-1-I don't see any reason for colonoscopy,for your age and history presented up till now.
This colonoscopy has better option to assess your anal / rectal condition, by proctoscopy which could be done in OPD under local anaesthesia by 2 % Xylocain Jelly.
So I stand by My opinion as in My last reply.

Q-2- or this is a seperate issue? is it still advisable to cancel the colonscopy.
Ans-2-This issue of thrombosed piles,is the cause of you having blood stains in 4th week, after piles surgery.
And So it is not advisable to go for Colonoscopy in this case , right now, in the given situation, as chances of damaging and flaring up of the proctitis and fresh bleeding from handling of the ano- rectal wound are more in such a case, which would complicate the healing anal wound.
So no intervention for at least 2 weeks of medicines and dietary treatment.

If you still want to go for Colonoscopy still,for want of worry,I would advise you to update Me the situation after 2 weeks time, when this issue could be assessed depending on the merrits at that time.
