What Causes Migraine And Dry Heaves After Cauterization For Bleeding Ulcer?

default
Posted on Fri, 8 Aug 2014
Question: 3 Months ago, I was cauterized for a bleeding ulcer. I was recovering nicely, when last Tuesday, I got a violent migraine, and started continuous dry heaves (I was NPO for an endoscopic exam). Then I lost total control of my bowels and was nearly losing consciousness. We had to send for an ambulance. I have no antigens in my blood and my stool culture (giardia) came up negative. I had been fighting diarrhea for 3.5 weeks. In the ER, my values were all good. There was no blood in my stool, even though my symptoms mimicked my ulcer hemorrhage. What happened?

Brief Answer:
Possible ulcer reactivation with no bleeding...

Detailed Answer:
Hi there,

I am sorry for the situation you are in.

You are saying that have been cauterized for bleeding ulcer 3 months back. I am happy that you were recovering nicely. This means that you did not have any bleeding episode and of course, you were treating your ulcer with medications.

Can you please mention what drugs did your doctor prescribe to treat your ulcer after cauterization?

Next, you are reporting to have fighting diarrhea for around 3.5 weeks. So, your diarrhea is being chronicized, which makes me think over:
- possible medications you were taking. If you have been on long course of antibiotics, they would have altered the intestinal flora, leading to disbacteriosis (which was treated with Flagyl-that stopped your diarrhea)
- ulcer reactivation with no bleeding
- IBS
- liver/gallbladder problems (as you are experiencing continuous vomiting: at this point,

I ask if you vomit right after taking foods or not related to foods or any other activity?

If it is right after eating, do you remember if there is a particular food that triggers vomiting or is it everything you eat?)

To my opinion, should rule out organic problems first as no blood were noted and your stool culture came up negative:
- reactivation of ulcer: upper endoscopy will determine it
- local bowel/liver/gallbladder problems: colonoscopy, abdominal ultrasound

If everything will be fine, then, IBS should be considered.

As you were nearly losing consciousness, it was because of dehydration due to severe diarrhea. It is very important to keep well hydrated through:
- taking plenty of fluids during the day
- take oral rehydration salts added to water to replace lost electrolytes
- tasectan or pepto bismol for diarrhea
- probiotics to optimize intestinal flora

Hope it was of help!
