Question: Sir I had a possible exposure to HIV virus I notice red rashes on chest and arm low grade fever fatigue and pain in kness white tongue these starts with 23th day after exposure I am in fear I was tested on18th, 24th and 32 days with 4th generation p24 test combo (cmia) and it was negative sir help me I am in great depression and fear, I feel restless .I am from XXXXXXX
The rashes are red and pink which are slightly disappear and no itching I can't sleep I feal heat and sweat in night.

You have been tested with latest technique 4 times and all results are negative. Your symptoms are also not favoring HIV.

You seem to be suffering from anxiety only. Rashes on arm and chest are likely allergic reaction.

In that case short course of anti anxiety medicines like clonazepam or etizolam with reassurance counseling are requirement. This will remove your fear.

Sir I had condom break with a sex worker in XXXXXXX that was my first time and I am a lot afraid after that incident I have fear of cold . I always looking for more rashes as they increase my anxiety sir only thought which came in my mind is that may be I may tested in window period I am feeling weak. I read all days about HIV tests and there accuracy . I read on one website that 4th generation test is 90% reliable when tested on 4weeks after that I feel more depressed all time negative thoughts are coming I can't able to contact the sex worker as se left the place this increase my anxiety more I feel little sore throats also please help me . I really regretting. I also do fever panel test which come out normal which also increase my fear and anxiety .I am In depression
What are the chances of do you think by seeing my symptoms and exposure . please help me from this trauma. Which test should I do i am worried about the window period some say3 month.

Thanks for information.

First of all there is chance of transmission only when that sex worker has HIV and that is the least likely possibility. I can understand your anxiety and we come across such type of young males with anxiety. I have seen 5 cases like you in person as well as online. None of them came positive.

Whenever you feel anxious take deep slow breathing. Inhale for 3 seconds, keep breath in for 3 seconds and exhale for 3 seconds. Do it for 10-15 minutes. You will feel relax.
Follow healthy lifestyle. This includes regular daily exercise (walking or running), hygienic food habits (no junk food, no fatty food, preferably only home-made food with law salt and fat with more fruits and vegetables) and timely scheduled sleep. Keep yourself busy.

Avoid reading about HIV as this will create more anxiety.

You can wait for completion of 3 months and can do last test just for assurance at end of 3 months. For a while consulting psychiatrist or your physician and taking anti anxiety drugs will be helpful.

How reliable is my 4th generation test with p24 antigen
And do p24 look for HIV 2 also? Sir I know you there is very less chance approx 1% but this still make me breathless and the symptoms increase my anxiety to more high level we are in 2016 don't we have a 100% surity about the HIV I tested negative on 31st day give me little strength but still I can't come out this do you see any case which come positive after negative 4th generation test on 28 day??Also sir every time my p24 index value came out to be 0.07 means 3 time I have tested and the value remain the same . I did the test from Dr lal path lab

Hi,

p24 antigen combi test is highly reliable more than 95%. Generally it is used after 4 weeks as after this time antibodies are completely formed. Your chances are even less than 0.1%. It cannot detect HIV-2. However in XXXXXXX HIV 2 is not prevalent, it is there in XXXXXXX

No test will come positive even after 3 months. There are many things in world for which we cannot give 100%.

Being a psychiatrist I do not come across the patient who come for me diagnosis or treatment, they come for either depression who have already HIV or sent to us when they have fear of HIV but in actual they are tested negative. So I have not seen any case who comes positive after negative on 28th day.

As you are not infected your index value is not going to change. Try to think actively that you are not infected and continue relaxation techniques.
