What Causes Low BP After Having Elantan?

default
Posted on Sat, 3 Jan 2015
Question: I have used medication as suggested by you. I was put on elantan 50 la. 75 mg atenalol i seem to be ok with an acceptable bp until i take elantan. Bp then drops to 102/62 and lower. Bp last taking was 129/79 p 67. And i still have to take. Pharmapress for night. Feel "ok" now. In morning i woll be ok until i take elantan then the cycle of low bp starts again for whole day . This is aweful with head spinning. Internist says heart ok
Blood tests ok. Arteries"open" one said no blockage after test 3 years ago. Other says without any test bar echocardiogram there MUST be some blockage. At least 15%. But heart healthy. Cannot understand. Is elantan too strong? Only that available or else elantan 20 tabs by state.

Brief Answer:
Further Dose reduction is mandate in your case.

Detailed Answer:
Hi Sir.

Good to have you back.

I understand your concern.

You still seem to have postural hypotension.
This necessitates further modification in the drug dosage.

The large contributor nocturnal drop of blood pressure in your case is Elantan 50 LA.

I strongly recommend the dose of Elantan LA is brought down to 25mg once day at bedtime.

Similarly Atenlol 75 mg can be brought down to 50 mg to be taken in the morning, followed by the prescribed dose of Pharmapress in the afternoon.

Avoid additional BP lowering drugs in the night except Elantan LA 25mg.

Please note all these drug dose modifications have to be taken only under strict medical supervision.

A Coronary angiogram is needed to rule out blockades in the heart.
Echocardiogram is not sensitive for this purpose

Thank you.

Brief Answer:
Avoid Lasix.Pharmapress once daily enough

Detailed Answer:
Hi Sir.

Yes sir ,you can open this discussion at your ease after 3 days.

I strongly advise you to avoid Lasix as it can contribute to serious electrolyte imbalances along with low blood pressure in the elderly.

You can take pharmapress once daily since amtas2.5 is being taken twice a day.

Elantan can be taken twice a day at 20 mg dose.

Do not make trials with Lasix.

The above prescription should suit you the best.

Post your further queries,if any.
Thank you.

Hi doctor. I took up your suggestion with gp. I am now on elantan 20x twice daily. 2.5 amtas x twice daily. I have been told to take pharmapress 10 x twice daily. For stress i was put on a medication which throws a spanner in my waterworks making it hard to pass urine. I was then given udiflpw but stopped using stdess med. I hace prostate problems which i think is a making of years of lasix use. Aftee taking lasix i have udinating probs until lasix iz worked out. Fortunately kidneys still ok despite high dosage of lasix. I find that the med as taken now still brings bp low. Pulse runs at 66/58 whoch i see as low. I have tried the following. Tahing elantan and amtas at half tab each morning. Half dose pharmapress abojf 15h00. Thos seems to work daytime
Evening half amtas.and as safety measure 20 mg elantan. Bp reasonable during night ggoing to 140/ 85. I am trying this out. Bp 130/74 range. I am keeping record to take to gp in 14 days tome for bp ecaluation. I get geasacges a side effect of uriflpw. Stress tabs are bazspam. Your comment please. Seems drs hsre are guessing. Will be posteng more after your commeny. Your assistance to here invaluabli.

Brief Answer:
Avoid lasix and Get Prostrate ultrasound.

Detailed Answer:
Hi Sir.

I understand your concern.

The current dose modifications as per your needs are are quite scientific and can be followed as such .

BP at 130/74 seems appropriate for your age right now.

Avoid lasix to prevent electrolyte and BP fluctuations as far as possible.

Half the scheduled doses can be followed as long as the BP is the range of 130-140/70-90.

Avoid overcorrection.

Thank you.
