What Causes Light Headedness After Focusing On A Nearby Object?

default
Posted on Thu, 12 Mar 2015
Question: My head swirl and I have an out-of-body experience (like a drop on a roller-coaster) during exercises to test for accommodation on my eyes. Every time, I have to shift my eyes to the left upper plane, I burst out I crying. This also happens if I have to focus my eyes on a near object such as my thumbs placed in front of me. The crying ceases once the exercise changes. I also get this swirly out of this world feeling when I walk and move my head up and down in an area where there are minute floor patterns, such as tile or parquet. What are some of the possible differential diagnoses?

Brief Answer:
Diseases of the inner ear and vertigo needs to be ruled out.

Detailed Answer:
Hello

Thank you for the follow up question.

I am very sorry as answer for the previous question was sent as I had pressed the back button because of connectivity problems.

I reviewed your history.
From the symptoms you have mentioned the likely differential diagnosis I would like to consider are
1. Benign positional vertigo
2. Eye problems like acquired nystagmus
3. Cervical spinal cord problems like spondylosis
4. Neurological problems
5. Diseases of the inner ear like Meniere's disease

If I were to be treating doctor I would like to start with symptomatic treatment with anti-vertigo drugs like Cinnarizine.

If the symptoms still persist then I would like to advice you ENT examination and neurological examination.

Hope this answers your question and the information is helpful to you.

Thank you
With regards

Apology accepted. Thanks for your response. At least, I will have a number of options to explore when next I see my neurologist. I was prescribed Meclizine but have not yet started it due to fear of drowsiness.
