Question: What can cause a 3 year old to have a normal platelet count but have large platelets?

Based on your query, my opinion is as follows:

1. Large platelets are usually a feature of reactive change like in low platelet count or inflammation.
2. As the platelet count is normal, it might be in response to an infection.
It has no value with normal platelet count and has no role in inflammation or infection. Its only a reactive response as other white blood cells are increasing.
Children have rapidly responding bone marrow. Platelet can be large sometimes normally too.
