What Causes Joint Pain In A Patient Suffering From Mild Osteoporosis?

Question: Hi Doctor,
I am writing this on behalf of my mother. She is aged ~53 and weight of about ~75-80. height of ~5.3-5.4. Recently she started having trouble in her knees. On initial diagnosis, the doctor said it was mild case of osteoporosis. The current level of symptoms include knee/foot join pain. More on the left leg than the right. The doctor mentioned that the lubricant between joints had become a bit dry. The pain is moderate to less on the morning and get gradually more towards the end of the day. Find it difficult in using XXXXXXX toilets, standing and sleeping at night is somewhat painful. Although the doctor said its a mild case, and recommended some XXXXXXX ( Prescription attached along ), I am still very concerned and really want to understand the problem deeper and make every possible effort to get better and fight the problem. I dearly wish that you recommend
- Strict do's and dont's in everyday activities
- Cross inspection of the prescribed medication, possible side effects considering the age as well.
- Natural home treatments for betterment
- Any alternate line of treatments
- Restrictions on travel, lifting luggages, household chores, stting down cross legged ?
- Any other information you require for me to provide in assisting of the diagnosis ?

Hi there.

It appears that she has not only Osteoporosis or weak bones but also Osteoarthritis of the knee.

This is age related degenerative changes on the joints. Avoid squatting, cross leg sitting, climbing stairs, kneeling. Use western toilet and avoid using XXXXXXX latrine system. Avoid lifting weights. She can wear knee cap for both knees.

Apply hot packs to the painful regions. Do physiotherapy to strengthen the muscles around the knee. Eat diet rich in calcium and Vitamin D like eggs, cheese, fresh fruits and green leafy vegetables, chicken, lean meat, soya, raagi.

She can take Tablet Calcium and Vitamin D3 supplements like Tablet Shelcal CT once a day for a month. This can be taken along with XXXXXXX The above medication can be taken once your prescription Medicine is over. No size effects, don't worry.

Thanks for the advice. Need some clarifications on the below lines -

- Do u have some diet suggestions based on a typical south XXXXXXX rice based vegetarian diet ?? - I will surely put her on raagi diet

- Could you suggest what kinds of physiotherapy treatments are helpful in this situation ? Any particular movement / mobilization/stretching techniques that I can just look up on the web for the time being and teach her ??

- What kinds of stretching are totally prohibited for her to be doing ?? For instance I know that she does XXXXXXX namaskaraa and bending down and touching feet and stting on the floor with legs straight out and extending arms to touch the toes ? Would these worsen the problems ??

- Since I know from my personal low back problems in the past, Immobility or lack of activity will only make it worse than before. Hence I am extremely inclined towards her doing some stretching everyday and not just take rest. So I wonder what are the safe and not safe kind of exercises/stretches that she can perform daily ??

- Do you recommend some additional tests ? lIke checking on calcium, d3 or other vitamin levels ?? or thyroid or cholesterol or bp checks or bone density ?? She has thyroid problems from 5-6 years now and has been taking medicines daily. Can that be a cause of the problem ?? Should we recheck the thyroid levels ?

Hi there.

She can consume brown/red rice which has more fibre and less carbohydrate. Ragi is a good idea. Soya must be added. Paneer and Yoghurt or Lassie will help with improving calcium and Vitamin D levels. Also plenty of fresh fruits and green leafy vegetables to be taken. Avoid ghee, vanaspati.

She can do the exercises you have mentioned as long as she isn't uncomfortable doing them. Also keeping the leg straight and pressing her knee by herself to the bed and holding for 20 seconds at a time is good. She should also move ankles up and down regularly. Yoga is very beneficial. She should avoid bending the knee much. She may do cycling on Exercise cycle machine which is extremely useful to keep fit and also does not harm the knees.

She can get her Thyroid levels checked every 6 months to see if they are in normal range. She's can get tested for Cholesterol, LDL, VLDL, Random blood sugar. She can get a DEXA Scan to look for soft and weak bones. For the arthritis she can get an injection called Injection Hyalgen-1. It can cost upto Rs. 12000 to 15000 which needs to be injected once into the knee and will provide good relief from pain for about a year. this is better than taking long term medication because very less or no side effect.

Thyroid problems can cause obesity which can increase the severity of arthritis quickly hence controlling Thyroid levels and losing weight is very much important for her to get relief from Arthritis.
